El hematoma epidural espinal espontáneo es una causa infrecuente de compresión medular aguda. Se definen como aquéllos que ocurren en ausencia de traumatismo o yatrogenia (intervención quirúrgica, punción lumbar, raquianestesia). Presentamos un caso de localización dorsal, en que no se halló patología causal del sangrado. Pese al diagnóstico y tratamiento quirúrgico precoz (dentro de las primeras 12h), la evolución clínica no ha sido favorable, sin recuperación alguna del déficit neurológico completo que presentaba al ingreso.
