Uso de duramadre homóloga en enfermedades neuroquirúrgicas

Resumen

El presente trabajo prospectivo tiene como objetivo evaluar el uso de duramadre homóloga preservada en glicerina (DUMHP) en pacientes con enfermedades neuroquirúrgicas hospitalizados en el Servicio de Neurocirugía del Instituto Nacional de Salud del Niño, Lima- Perú, desde enero 1983 a diciembre 1989.

Se ha utilizado en: a) Laceración meníngea traumática; 24 casos; b) plastia de encefalocele: frontoetmoidonasal, 20 casos, y occipital 7 casos; c) plastia de mielomeningocele roto, 60 casos; d) craneoestenosis, 96 casos; e) disostosis craneofacial, 4 casos; f) plastia de lesión meníngea por compromiso tumoral-edema, 2 casos; g) plastia de lesión meníngea por cirugía de fosa posterior, 2 casos; y h) plastia de seno transverso parcialmente roto por traumatismo encéfalocraneano, 3 casos.

No se observó rechazo a este tipo de dura, ni complicaciones atribuibles a su uso en el seguimiento de 1 a 6 años, por lo que sugerimos la utilización de este elemento quirúrgico por ser barato, fácil de conseguir, no producir morbimortalidad y que pueda ser usado en situación de emergencia.

Summary

The purpose of the present prospective study was to analyze the use of homologous dura mater preserved in glicerine (DUMHP) in patients with neurosurgical diseases at the Neurosurgery Service of Children Hospital, Lima-Perú, since january 1983 to december 1989.

It has been used in: a) traumatic meningeal lacerations, 24 cases; b) repair of encephaloceles: frontoetmoidonasal, 20 cases, and occipital, 7 cases; c)repair of the open myelomeningoceles, 60 cases; d) craniostenosis, 96 cases; e) craniofacial disotosis, 4 cases: f) repair of menigeal laceration by tumoral expansion and edema, 2 cases; g) repair of meningeal laceration by infratentorial surgery, 2 cases; and h) repair of partially open transverse sinus by skull trauma, 3 cases.

We have not observed rejection of the dura mater graft or related complications during a follow up period of 1 to 6 years; therefore, we suggest the use of this surgical element because it is cheap, easy to obtain, low risk at morbimortality and the fact that it can be used on an emergency basis.
