Los hematomas de cerebelo representan una patología especial dentro de los hematomas intracraneales. Dadas las exiguas dimensiones de la fosa posterior determinan con frecuencia compresión precoz del neuroeje y de las vías de drenaje del líquido cefalo-raquídeo. Es por ello que un diagnóstico y tratamiento adecuados son vitales para obtener los mejores resultados clínicos. Aquí presentamos nuestra serie de 23 casos con la evolución seguida y los tratamientos aplicados, con la finalidad de evaluar la mejor actitud terapéutica a indicar según el estado del paciente y el tamaño del hematoma. Como conclusiones de nuestro estudio diremos que en los hematomas de tamaño comprendido entre 2.5 y 3.5cm de diámetro el drenaje ventricular suele ser suficiente para evitar el empeoramiento del enfermo por hidrocefalia, mieptras que el tratamiento de los hematomas de más de 3.5cm de diámetro debe incluir drenaje ventricular y la evacuación del hematoma mediante craniectomía suboccipital. A diferencia de los hematomas de los ganglios basales, la calidad de vida de los supervivientes suele ser buena en la mayoría de los casos, quedando con déficits neurológicos de poca magnitud (ataxia e incoordinación) pero son indispensables un diagnóstico y tratamiento adecuados.

Cerebellar haematomas are a special subgroup of intracranial haematomas. Due to the reduced dimensions of the posterior fossa they soon determine early compression of the brain-stem and CSF pathways. Thus a correct diagnosis and treatment are vital to archieve the best clinical results. Herewith we present our series of 23 cases together with their clinical course and the treatment applied, with the aim to analyse the best therapeutic approach to be recommended according to the clinical status of the patient and the size of the haematoma. We would summarise our results saying that in haematomas with a size between 2.5 and 3.5cm in diameter the ventricular drainage is usually enough to prevent the deterioration of the patient due to hydrocephalus, while de treatment of haematomas bigger than 3.5cm in diameter should include ventricular drainage plus haematoma removal through a posterior fossa craniectomy. Contrary to the haematomas in the basal ganglia, the quality of life of the survivors is usually good in the great majority of the cases, remaining neurological deficits of little entity (ataxia and incoordination), but is vital to do a correct diagnosis and treatment).
