Resumen

Los quistes de la bolsa de Rathke (QBR) son un hallazgo relativamente frecuente en autopsias y en estudios de tomografia axial computerizada (TC) y resonancia magnética (RM). Generalmente son asintomáticos. La diferenciación preoperatoria por TC y RM de otros tumores selares y supraselares es importante, porque el pronóstico y manejo quirúrgico es diferente para éstos quistes. Presentamos el caso de una paciente de 30 años, portadora de un quiste de la bolsa de Rathke sintomático diagnosticado preoperatoriamente con TC y RM, a la que se le realizó vaciamiento y resección parcial de la pared del quiste por vía transesfenoidal, y que presentó como complicación postoperatoria una fístula de líquido cefalorraquideo e importante neumoencefalo que precisaron reoperación.

Summary

Rathke's cleft cysts are a relatively common finding in MR imaging, CT scanning and autopsies, most of them being asymptomatic. Clinical and imaging identification of this condition may be important, because these peculiar cysts are not always managed in the same way as other tumors or occupying-processes of the opto-chiasmatic region.

A symptomatic cyst of Ráthke's clert, in a 30 yearold woman, is presented. The cyst was diagnosed by CT and M.R.I. and operated on by transsphenoidal route. Postoperative C.S.F. leakage and pneumocephalus accounted for a reoperation ten days after removal of the cyst.
