Resumen

Desde hace varios años, se viene practicando la embolización preoperatoria de los meningiomas con el ñn de reducir la hemorragia quirúrgica por taponamiento vascular, y facilitar su extirpación. En el presente trabajo, resaltamos la necrosis isquémica que la embolización produce en la masa tumoral transformándola en un tejido fácilmente aspirable. Ello es especialmente útil en meningiomas de algunas localizaciones de abordaje complicado. En nuestro caso, un voluminoso meningioma de ángulo ponto-cerebeloso fue resecado totalmente y con relativa facilidad a través de un abordaje convencional mediante craniectomía suboc-cipital lateral.

Se revisa la técnica, indicaciones y control de la embolización preoperatoria.

Summary

Preoperative embolization of meningiomas has been performed in order to reduce surgical hemorrhage during the removal of these vascularized tumors. In this paper we emphasize the fact that occlusion of the tumoral vessels by artificial emboli produces an ische-mic necrosis that greatly helps tumor exéresis. This is especially useful in meningiomas of certain localizations requiring complex surgical approaches. In our case, a giant ponto-cerebellar meningioma was dried up totally and then removed with relative ease through a conven-tional suboccipítal lateral craniectomy.

The technique, indications and control of preoperative embolization are reviewed.
