Tumor neuroectodérmico primitivo del nervio ciático

Resumen

Presentamos un caso de tumor neuroectodérmico primitivo originado en el nervio ciático, en un paciente con un cuadro clínico de ciatalgia intensa desencadenada siempre al sentarse y un abultamiento notable en la región glútea derecha. En la intervención quirúrgica, se identificó en el tronco del nervio ciático, en el trayecto situado ligeramente por encima del pliegue glúteo, una tumoración fusiforme de 8×12cm, que englobaba el nervio. Histológicamente, el tumor estaba constituido por células neoplásicas de núcleos redondeados, relativamente uniformes y escaso citoplasma. Junto a numerosas imágenes de mitosis, estaban significativamente representadas estructuras rosetoides, en las que las células neoplásicas se disponían en corona con centros fibrilares (Rosetas de Homer-Wright). Con M. E. se observaron células pobremente diferenciadas, de pequeño tamaño, con escasas prolongaciones citoplasmáticas, raros microtúbulos y aislados gránulos electrodensos. Con técnicas de inmunohistoquímia, se detectó inmunoreactividad débil para NSE y moderada y zonal para sinaptofisina. En conclusión, pensamos que se trata de una neoplasia maligna de nervio periférico, rara, originada a partir de células neuroepiteliales primitivas, con características microscópicas, ultraestructurales e inmunohistoquímicas similares a las de los Tumores Neuroectodérmicos Primitivos del Sistema Nervioso Central.

Summary

A case of primitive neuroectodermal tumor arising in the sciatic nerve of a 30-year-old man is presented. The patient was admitted to hospital with a sudden right-side sciatica and a tumoral mass in the right buttock. At surgery a fusiform tumor of 8×12cm was found in the proximal portion of the sciatic nerve. An en block scission of tumoral mass with the segment of sciatic nerve affected was done. The histological examination revealed a tumor composed of small cells with scanty cytoplasm and darkly-stained nuclei with fairly frequent mitoses. In many areas the tumor cells were arranged in a well-marked rosette pattern with pale eosinophilic cores (Homer-Wright rosettes). Electron microscopy showed poorly differentiated cells with few cytoplasmic processes, rare microtubules and isolated dense-core granules. On immunohistochemical study, a weak reaction with anti-NSE and a zonal positivity for synaptophisin were found. We conclude that this tumor is a malignant peripheral neuroectodermal tumor derived from primitive neuroepithelio, with microscopical, ultrastructural imd immunohistochemical characteristics similar to Primitive Neuroectodermal Tumors of the Central Nervous System.
