Resumen

Presentamos una paciente de sexo femenino, de 33 años de edad, con ataques isquémicos transitorios (AIT) ocasionados por un aneurisma gigante parcialmente trombosado de la arteria cerebral media derecha. Se intervino quirúrgicamente practicándose anastomosis término-terminal de arterias temporal superficial y cerebral media derecha, y exéresis total de la lesión aneurismática. El resultado postoperatorio fue satisfactorio, con una exploración neurológica normal. La angiograña cerebral postoperatoria, demostró la desaparición del aneurisma y permeabilidad de la anastomosis vascular extra-intracraneal.

Summary

We report a case of a partially trombosed giant aneurysm of the right middle cerebral artery (MCA) in a 33 years-old female suffering transitory ischemic attacks (TIAs). The diagnosis was established by CT scan, MRI and cerebral angiography. Surgical treatment was performed, the aneurysm was totally resected and the distal circulation in the MCA territory mantained by a termino-terminal superficial temporal artery (STA) by-pass.

The patient remained asymptomatic until the last follow up 6 months after surgery. The STA-MCA by pass was patent in the follow up angiographic control.
