Meningiomas petroclivales gigantes

Resumen

Se trata de 4 pacientes, tres mujeres y un varón, diagnosticados de meningiomas petroclivales gigantes. En todos los casos superó los 44mm de diámetro mayor. La clínica predominante fue la hipertensión endocraneal con afectación de pares craneales. El abordaje utilizado fue el transcoclear modificado. Se practicó pretrosectomia radical y se empleó la vía presigmoidea para la apertura dural. Se preservo el seno sigmoideo y transverso. La tumoración fue extirpada de forma total (2 casos), subtotal (1 caso) y parcial (1 caso). No hubo mortalidad operatoria (éxitus hospitalario). Existió morbilidad neurológica relacionada fundamentalmente con los nervios craneales V-VI-VII-VIII y un caso de hemiparesia.

Se detallan los aspectos y las bases técnicas de la técnica quirúrgica empleada. Se contrastan los resultados obtenidos con otras alternativas más clásicas. Finalmente, concluimos que los meningiomas petroclivales son entidades quirúrgicamente tratables, aunque siempre reclamarán abordajes dificultosos y una técnica microquirúrgica meticulosa.

Summary

Four patients with giant petroclival meningioma are presented. In all cases the tumor size was over 44mm. The main clinical symptoms were related to raised intracranial pressure. Cranial nerve deficits were observed in almost all cases. A modified transcochlear approach in two stages was used. Total tumor removal was achieved in one case and the remaining patients had partial removal. There was no mortality and complications were related to cranial nerve dysfunction.
