Siringohidromielia terminal aislada como causa de paraparesia

Resumen

Presentamos un caso clínico de paraparesia de larga evolución en una mujer de 75 años, diabética insulinodependiente, cuyo déficit motor fue atribuido inicialmente a una polineuropatía diabética. Un estudio neurofisiológico orientó a un síndrome de cola de caballo o cono medular que fue confirmado mediante técnica de resonancia magnética, poniendo de manifiesto la existencia de una cavidad quística en cono medular, cuyo resultado quirúrgico e histológico fue el de quiste neuroepitelial sin otras lesiones asociadas. Comentamos lo inusual de esta forma de presentación de cavidades siringohidromiélicas terminales, su diagnóstico diferencial y manejo quirúrgico.

Abstract

We are reporting a clinical case of paraparesis in a 75 years old woman. Paresis was initially attributed to diabetic polineuropathy.

Neurophysiological studies suggested a lesionof the medullary cone or the filum terminale which was confirmed by a MR study showing a cystic dilatation of the medullary cone. The patient was operated and the histological study showed a neuroepitelial cyst.

The clinical presentation in our patient with terminal siringohydromielia is rather inusual.
