Resumen

Presentamos cuatro casos de aneurismas de la arteria vertebral-PICA, intervenidos en nuestro Servicio mediante un abordaje lateral extremo transcondilar. Nuestros pacientes, tres hombres y una mujer, tenían edades comprendidas entre 28 y 64 años. La hemorragia subaracnoidea fue la manifestación clínica inicial en tres de los casos y los síntomas y signos de compresión de estructuras adyacentes lo fueron en el restante. En nuestra serie no hubo mortalidad quirúrgica. Tan sólo un paciente, portador de un aneurisma gigante de la AV-PICA, presentó morbilidad por lesión permanente de pares craneales bajos. El abordaje lateral extremo transcondilar está indicado en el tratamiento quirúrgico de lesiones que asientan en la cara anterior del agujero magno y área del clivus bajo. Esta técnica permite el abordaje quirúrgico a los aneurismas de la AV-PICA, con una excelente exposición y mínima retracción de las estructuras nerviosas, frente al tradicional abordaje mediante craniectomía suboccipital unilateral.

Summary

Four cases of vertebral artery-PICA (VA-PICA) aneurysms, operated in our department through an extreme lateral transcondylar approach, are presented. Our patients, three men and one woman, were aged 28 to 64 years at presentation. Three of the cases presented with a subarachnoid haemorrage. Signs of brain stem compression were observed in one patient. There was no sugical related mortality in this series. Only one patient with a giant VA-PICA aneurysm suffered surgical morbidity from definitive lesion of lower cranial nerves. The extreme lateral transcondylar approach is indicated in the surgical treatment of lesions located in the lower clivus and anterior aspect of the foramen magnum. This technique allows an excellent exposure of VA-PICA aneurysms and reduces the retraction of nervous structures in comparison to the conventional suboccipital approach.
