Resumen

Se ha descrito un total de dieciséis casos de tumores sacros con extensión presacra que debutan como una lesión presacra. Nosotros deseamos presentar el caso de una mujer cuyos síntomas empezaron a la edad de 18 años. Acudió a nuestra consulta quince años después, tras haber sido sometida a una biopsia y a dos resecciones parciales.

La lesión se extirpó a través de una vía anterior transperitoneal, con desaparición de la sintomatología preoperatoria. La paciente requirió tratamiento urológico crónico por presentar hidronefrosis, así como una artrodesis lumbosacra. No se administró radioterapia postoperatoria. No han habido recidivas en cinco años de seguimiento.

Dado que estos tumores son histológicamente benignos, deben ser tratados de forma agresiva con la intención de conseguir una extirpación completa. La vía de abordaje quirúrgico depende del grado de destrucción del sacro, de la extensión intrapélvica y de la afectación de las articulaciones sacro-ilíacas.

Summary

Sixteen sacral tumours with presacral extension preseting as a presacral mass have been described in the literature. We are presenting the case of a female whose symptoms started at the age of 18 years. She carne to our elinic fifteen years later, after having undergone a biopsy and two partial resections.

The lesion was removed by an anterior transperitoneal approach, with disappearance of her preoperative symptoms, but she required long term urological treatment for hydronephrosis and a posterior lumbosacral fusiono No postoperative radiotherapy was applied, and no recurrence has occurred through a five years follow-up.

As these tumours are histologically benign, they should be managed aggressively with the aim of complete resection. The choice of surgical approach depends on the degree of sacral destruction, intrapelvic extension, and sacroiliac joint involvement.
