Aneurisma angiográficamente oculto demostrado con resonancia magnética y cirugía

Resumen

Se presenta un caso de aneurisma de comunicante anterior de 10mm de diámetro, diagnosticado con resonancia nuclear mgnética y oculto en el estudio angiográfico, en un paciente con sangrado repetido y trombosis de la carótida interna. Se propone sustituir el término de «aneurisma angiográficamente oculto» por el de «aneurisma oculto». Finalmente, se sugiere introducir en el estudio etiológico de la hemorragia subaracnoidea con angiografía inicial negativa un estudio RNM y angio-RNM o de angio-TAC helicoidal precoz.

Summary

A case report of a 10mm anterior communicating aneurysm non visualized after an angiographic study and showed with MRI is described in a patient with recurrent hemorrhage and carotid artery occlusion. The authors propose change the term «occult angiographic aneurysm» by «occult aneurysm». Finally, they suggest include in the diagnostic work-up of the subarachnoid hemorrhage an early MRI and angio-MRI or Angio-CT helicoidal scan study in the event of a negative angiographic exploration.
