Mucormicosis cerebral aislada: a propósito de un caso y revisión de la literatura

Resumen

Con el término «mucormicosis» se designan aquellas infecciones producidas por hongos del orden de los mucorales. La enfermedad por lo general se desarrolla en pacientes inmunodeprimidos o en situación de acidosis, manifestándose como una infección habitualmente rino-órbito-cerebral. No ha sido establecido un síndrome clínico que sugiera esta afección.

Presentamos el caso de un varón de raza negra, de 32 años de edad, que tras sufrir una crisis convulsiva generalizada, se apreció, mediante técnica de neuroimagen, una lesión ocupante de espacio ubicada en el polo del lóbulo frontal izquierdo, que en principio fue etiquetada de absceso cerebral. En ningún momento se constató que el paciente sufriera un cuadro febril ni antes de su ingreso ni durante el mismo. Por otro lado, no padecía enfermedad inmunodepresiva y no presentaba adicción a drogas.

Previo tratamiento antibiótico y antiepiléptico convencional, que no modificó el tamaño de la lesión, se practicó cirugía exerética de la lesión nodular que fácilmente se delimitó de los tejidos cerebrales vecinos.

Son únicamnte once los pacientes afectos de «mucormicosis cerebral aislada» descritos en la literatura mundial que evolucionarion satisfactoriamente. Aportamos un nuevo caso de esta rara y a menudo fatal afección.

Abstract

The term «mucormycosis» designates infections produced by fungi of the mucor strain. The illness in general develops in immuno-depressed patients or in a situation of acidosis, manifesting itself normally as a rhino-orbital-cerebral infection. A clinical syndrome suggestive of this disease has not yet been established.

We present the case of a 32 year-old black male patient. Who suffered a generalised seizure, and showed a space occupying lesion located at the pole of the left frontal lobe. Which was initially labelled as a cerebral abscess. The patient did not have fever-neither before being admitted nor during hosdpitalisation and did not suffer from an immuno-depressive disease on addiction to drugs.

After treatment with antibiotics and conventional anti-epileptic treatment, which did not change the size of the lesion, radical remoral of the nodular lesion which was easily delimited from the neighbouring brain tissue was carried out.

Only eleven patients suffering from «isolated cerebral mucormycosis» described in world literature have recovered satisfactorily. We add a new case of this rare and frequently fatal disease.
