Resumen

Se presenta un estudio prospectivo de 43 pacientes con hernia discal cervical espondilótica, intervenidos quirúrgicamente por el abordaje anterior con injerto óseo. Nuestros resultados obtenidos siguiendo los criterios de Odom fueron excelentes en el 65% de los casos, buenos en el 27%, aceptables en el 4.6% y pobres en el 2.3%. Con el fin de detectar posibles factores pronósticos hemos correlllcionado los resultados con la edad, tiempo de evolución de la enfermedad y el estado neurológico inicial. Hemos encontrado que edades bajas (<40 años) y tiempos de duración de la enfermedad (<2 años) eran factores pronósticos posiblemente favorables. Los pacientes con déficit radicular han tenido un resultado superior a los pacientes con déficit piramidal o mixto.

Summary

We report a retrospective study of 43 patients with spondylotic cervical herniated disc, treated surgically by the anterior approach with bone grafting. Our results which were achieved according to Odomo's Criteria were excellent in 65% of the cases, Good in 27%, Fair in 4.6% and Poor in 2.3%. For detection of possible prognostic factors we correlated our results with the age, the clínical course, and the initial neurological stage. We found that a lower age (<40 years) and a shorter duration of the illness (<2 years) were positive prognostic factors. We found that patients who initially had radicular type déficit showed more dramatic improvment postoperativly than those with pyramidal and or mixed type of déficit.
