Motor vehicle accidents, occupational accidents, and interpersonal trauma and falls are the most common reasons for head trauma, especially among the young population. Traumatic brain injury leads to negative social and economic consequences because of its mortality, and extended and complicated period of recovery.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Following the primary brain injury, metabolic and inflammatory alterations, oxidative stress, and vasospasms trigger the secondary insult cascade, resulting in cerebral edema and intracranial hypertension.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">1</span></a> The primary goal in the management of TBI is to prevent a secondary insult by avoiding hypotension, hypoxia, intracranial hypertension, and maintaining adequate cerebral blood flow.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">2</span></a> In order to achieve this goal, sedation and analgesia in the intensive care unit (ICU) play a key role. Barbiturate coma therapy has been proposed to control intracranial hypertension, optimize cerebral perfusion pressure (CPP), and prevent seizures in refractory cases when other treatment options have failed. In spite of these favorable effects, the benefit of barbiturate therapy has been questioned for its negative hemodynamic effects.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">3</span></a> Another under-recognized adverse effect of barbiturate treatment is severe dyskalemia: hypokalemia presents with the initiation of barbiturate therapy, and rebound hyperkalemia subsequently develops after the cessation of the therapy.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Given that the only available literature is a few case reports, we aimed to evaluate the incidence of severe potassium disturbances during barbiturate coma therapy in patients with severe TBI, and the characteristics of these patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study population</span><p id="par0020" class="elsevierStylePara elsevierViewall">This observational study was approved by the local ethics committee (Hitit University, Decree #2019-69). Severe TBI is defined as a Glasgow Coma Scale (GCS) score of ≤8.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">2</span></a> Accordingly, 37 consecutive patients with severe TBI who were treated with barbiturate coma in the intensive care units (ICU) of two hospitals (Kocaeli State Hospital and Hitit University) between 2015 and 2017 were analyzed in 3 groups [normal potassium levels (3.5–5.5<span class="elsevierStyleHsp" style=""></span>mEq/L), mild-to-moderate hypokalemia (2.6–3.5<span class="elsevierStyleHsp" style=""></span>mEq/L), and severe hypokalemia (<2.5<span class="elsevierStyleHsp" style=""></span>mEq/L)]. Patients who died in the first 24<span class="elsevierStyleHsp" style=""></span>h following admission to the ICU, patients with mild and moderate TBI (GCS >8), and patients who received thiopentone infusion for other reasons (e.g. non-traumatic ischemic or hemorrhagic disease, and status epilepticus) were excluded from the study. Data were acquired from the hospitals’ electronic data processing systems and daily bedside sheets.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Management</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Prehospital management</span><p id="par0025" class="elsevierStylePara elsevierViewall">All patients with head trauma were assessed and treated on-scene by a prehospital emergency medical team and the patients were immediately transferred to hospitals that had neurosurgical facilities. All patients with severe head trauma were intubated (whether on-scene or in the emergency room) and mechanically ventilated.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Emergency room and surgical management</span><p id="par0030" class="elsevierStylePara elsevierViewall">Computed tomography (CT) was performed as soon as possible after the patients were stabilized. All patients were evaluated by a neurosurgeon and the decision for a primary emergency decompressive surgery was made according to the patient's physical and medical condition by taking the benefits and risks of surgery into consideration. Primary decompressive craniectomy was reserved for evacuation of acute subdural hematomas and for removal of brain contusions with mass effect. Secondary decompressive craniectomy was performed as a last tier intervention in patients with refractory intracranial hypertension despite maximal first tier intracranial pressure (ICP) lowering interventions. Surgery was not performed in patients with diffuse brain injury (without a mass lesion) or in those for whom decompressive craniectomy was considered to be futile.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">ICU management</span><p id="par0035" class="elsevierStylePara elsevierViewall">All patients with severe TBI were followed up in the ICU. The treatment of patients with severe TBI in the ICU were performed in accordance with the recommendations of guidelines if circumstances allowed.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">2,4</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">A central venous indwelling catheter and a radial artery cannula was placed in all patients. Systolic arterial blood pressure was maintained above 110<span class="elsevierStyleHsp" style=""></span>mm Hg and hypertension was prevented. In order to achieve the minimum threshold of systolic blood pressure, hypovolemia was corrected with crystalloids, sedative agents were infused continuously (with a minimum dose that enables deep sedation), and norepinephrine was initiated if needed.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Ventilation therapy included normal ventilation or mild hyperventilation. Target thresholds were to maintain normocapnia (PaCO<span class="elsevierStyleInf">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>35–45<span class="elsevierStyleHsp" style=""></span>mm Hg) or a mild hypocarbia (PaCO<span class="elsevierStyleInf">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>30–35<span class="elsevierStyleHsp" style=""></span>mm Hg), and to prevent extreme PaCO<span class="elsevierStyleInf">2</span> values. The arterial partial pressures of CO2 were monitored with end-tidal CO2 and blood gas samples. The primary target for ICP was <25<span class="elsevierStyleHsp" style=""></span>mm Hg, and for CPP was 60–70<span class="elsevierStyleHsp" style=""></span>mm Hg. For the control of elevated ICP, patients underwent surgery (decompressive craniectomy, craniotomy or removal of brain contusions with mass effect) according to the neurosurgeons’ decision. The first-line medical treatment of raised ICP included mannitol 20% and deep sedation. Barbiturate infusion was considered as a second-tier medical treatment when ICP was failed to be controlled with other therapies. Phenytoin was also administered for prophylaxis of early seizures. None of the patients included in this study were cooled using either head-only or systemic methods. Steroids were not used for reducing ICP.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">5</span></a> Brain and cervical CT scans were performed without delay and repeated if needed.</p><p id="par0050" class="elsevierStylePara elsevierViewall">All patients’ hemograms, biochemistry, and blood gases were analyzed routinely on admission. Hemograms and biochemistry were evaluated daily. Blood gases were analyzed 4 times per day. All analyses were repeated when needed. Severe hypokalemia was treated with intravenous and enteral forms of potassium preparations. Severe hyperkalemia was treated with nebulized albuterol and intravenous calcium. If the initial therapy did not succeed and arrhythmias were observed, insulin with glucose was used as a last resort.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Statistics</span><p id="par0055" class="elsevierStylePara elsevierViewall">After confirmation of the normal distribution of the data using the Shapiro–Wilk test, one-way analysis of variance (ANOVA) was performed to analyze the significant difference between 3 groups of patients according to their potassium levels. Student's <span class="elsevierStyleItalic">t</span>-test was used for calculating the difference of the mean potassium supply per day during thiopentone therapy between patients with moderate-to-mild hypokalemia and severe hypokalemia. The Chi-square test was used to compare the categorical variables. Continuous data are presented as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>standard deviation, and categorical data are presented as counts and percentages. <span class="elsevierStyleItalic">p</span> values<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 were considered statistically significant.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">A total of 37 patients with severe TBI who were treated with barbiturates were evaluated. Among these, 17 patients developed mild-moderate hypokalemia (2.6–3.5<span class="elsevierStyleHsp" style=""></span>mEq/L), and 6 patients developed severe hypokalemia (<2.5<span class="elsevierStyleHsp" style=""></span>mEq/L) following the induction of the barbiturate therapy. No potassium disturbance was seen in 14 patients. The incidence of mild-to-severe hypokalemia following the initiation of thiopentone infusion was 62.2%. The rate of severe hypokalemia in patients with severe TBI who were treated with thiopentone was 16.2%. There was no statistically significant difference between the 3 groups in demographic characteristics, need for surgery, mortality rates, ICU length of stay, and duration of thiopentone therapy. Patients with normal potassium concentrations received no additional enteral or intravenous potassium supplies. The mean potassium supply per day during thiopentone therapy was statistically significantly different between patients with mild-to-moderate hypokalemia and those with severe hypokalemic (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">When the patients with severe hypokalemia were analyzed, it was observed that 4 of the 6 patients developed rebound hyperkalemia exceeding 6<span class="elsevierStyleHsp" style=""></span>mEq/L following the cessation of barbiturate therapy. All 6 patients were males and the mortality rate was 66.7%. The Acute Physiology and Chronic Health Evaluation (APACHE)-II scores of the 2 survivors were lower than the other 4 patients. The GCS of the 2 survivors on admission were the highest scores of all 6 patients (Case#1: 8, Case#4: 7). Of these 2 patients, the Glasgow Outcome Scale (GOS) score on discharge and the extended GOS (GOS-E) at one year after the trauma were 5 and 8, respectively. The patients were able to return to their educational lives one or two years after the trauma. The analysis of patients with severe hypokalemia is presented in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Thiopentone therapy was maintained with the lowest dose that provided deep sedation and immobility. The nadir potassium concentration was 1.5<span class="elsevierStyleHsp" style=""></span>mEq/L and the highest potassium value was 6.8<span class="elsevierStyleHsp" style=""></span>mEq/L. The mean time to reach nadir potassium concentrations was 2.8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.8 days (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). Case #1, case #3, and case #5 developed electrocardiogram changes due to hyperkalemia and received insulin–glucose treatment. Although there was a severe potassium disorder, plasma potassium concentrations returned to normal levels (3.6–5.5<span class="elsevierStyleHsp" style=""></span>mEq/L) after the cessation of barbiturate therapy. None of the patients encountered any life-threatening cardiac complications due to severe dyskalemia.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">As a main finding of this study, the incidence of mild-to-severe hypokalemia was 62.2% and the rate of severe hypokalemia was found as 16.2% following the initiation of thiopentone infusion in patients with severe TBI. Severe hyperkalemia (<span class="elsevierStyleItalic">K</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>6.5<span class="elsevierStyleHsp" style=""></span>mEq/L) following hypokalemia was observed in patients who received higher doses of exogenous potassium.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Barbiturates cause electroencephalogram (EEG) slowing, generalized reduction of excitatory synaptic inputs, burst suppression, and isoelectric activity.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">6</span></a> They show their neuroprotective effect by decreasing cerebral metabolic rate of oxygen (CMRO<span class="elsevierStyleInf">2</span>), mitigating secondary cell damage, and suppressing the excessive production of free radicals.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">3,7,8</span></a> Despite these neuroprotective features, they also have adverse effects including immunosuppression, hepatic dysfunction, and renal dysfunction.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> Additionally, high-dose barbiturate therapy is known to reduce arterial pressures.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a> In this study, no hepatic or renal dysfunction was observed, and 3 patients required vasopressors during thiopentone therapy. Regardless of their negative effect on hemodynamics, we were able to achieve the mean arterial pressure goals.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Severe dyskalemia refractory to treatment is a rare and under-recognized adverse effect of barbiturate coma therapy.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">11–16</span></a> Underlying disease, hypothermia, the sympathetic response to acute trauma, and drugs such as mannitol, diuretics, vasopressors/inotropes, insulin, and resuscitation fluids that are used in the ICU can contribute to dyskalemia, yet hypokalemia was thought to be directly related with the severity of trauma.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">17,18</span></a> Thus, we only included patients the severe TBI for standardization in this study, and patients who were treated with barbiturates for other reasons (e.g. status epilepticus, non-traumatic ischemic or hemorrhagic events) were excluded. It was reported in a study evaluating the adverse effects and complications of barbiturate therapy in patients with severe TBI that the incidence of mild-to-severe hypokalemia was 82% (31 of 38 patients).<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">19</span></a> In another study, the incidence of mild-to-severe hypokalemia in all thiopentone-treated patients (with any diagnoses) was found as 89.4% (42 of 47 patients).<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a> It was also observed in these studies that 8 of 38 patients (21%)<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">19</span></a> and 11 of 47 patients (23.4%)<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a> had serum potassium concentrations below 2<span class="elsevierStyleHsp" style=""></span>mEq/L during barbiturate therapy. The hypokalemia rates of the present study were found to be lower than in these previous studies, which may be the result of using the minimum effective doses of barbiturates in our clinic. Additionally, there was no significant difference in mortality rates of normokalemic and hypokalemic severe TBI patients. The whole mortality rate of all severe TBI patients in the study (48.4%) was similar to the previous studies.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Potassium is the main cation of intracellular fluid. One of the proposed mechanisms for barbiturate-related severe hypokalemia is intracellular potassium shift.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">21,22</span></a> On the other hand, the pH of 2.5% thiopentone solution in 0.9% sodium chloride is 10.6.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">23</span></a> The alkalization of blood may cause potassium influx into the cells. Another effect of thiopentone is the inhibition of voltage-dependent K channels in a concentration-dependent manner.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">24</span></a> Accordingly, it was observed in a study that hypokalemia occurred in patients who received higher doses of thiopentone.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a> It is known that excess mineralocorticoid concentrations and a catecholamine surge can cause hypokalemia. Another proposed mechanism for hypokalemia in patients with trauma is the increase in β2-adrenoreceptor stimulation due to sympathetic response, resulting in an activation of the sodium-potassium pump.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">25</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Regarding the knowledge that the insulin secretion from the β-cells of the pancreas affects the ion transport, barbiturates (especially thiopentone) were documented to significantly inhibit adenosine triphosphate (ATP)-sensitive potassium channels in increasing doses, leading to insulin secretion.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">26</span></a> All of these effects of barbiturates were shown to be reversed immediately with the wash-out of the drug.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">26</span></a> Following the cessation of barbiturates, when all the afore-mentioned effects that caused intracellular potassium shift has ended, outward potassium current will be observed. We can conclude with this present study that excessive and aggressive potassium replacement during the period of hypokalemia, or the abrupt cessation of barbiturate therapy would lead to rebound hyperkalemia. However, this study does not explain why some patients develop severe hypokalemia while others do not. Although there are some known risk factors which were mentioned previously, we cannot exactly determine the patients who are at high risk for developing barbiturate-induced severe hypokalemia. Because the cause of hypokalemia during barbiturate therapy can be explained by redistributive hypokalemia, in which the risk of hyperkalemia should be expected. Accordingly, mild hypokalemia (the lowest level that does not produce cardiac arrhythmias) may be permitted. Weaning barbiturates over time may gain time for the management of electrolyte imbalances. If the mechanism of hypokalemia during barbiturate therapy is well known, we could speculate that some treatment alternatives such as glucagon could be planned for reversing this adverse effect in the future.</p><p id="par0100" class="elsevierStylePara elsevierViewall">The strength of this study is the good standardization of the patients. The limited number of patients and its retrospective nature are the limitations of the study.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusion</span><p id="par0105" class="elsevierStylePara elsevierViewall">Severe hypokalemia refractory to medical treatment and rebound hyperkalemia is a serious adverse effect of thiopentone coma therapy in patients with severe TBI. Excessive and aggressive potassium replacement during the barbiturate-induced hypokalemia period must be avoided. Seventeen patients developed mild-moderate hypokalemia (2.6–3.5<span class="elsevierStyleHsp" style=""></span>mEq/L), and 6 patients developed severe hypokalemia (<2.5<span class="elsevierStyleHsp" style=""></span>mEq/L) following the induction of barbiturate therapy. The incidence of mild-to-severe barbiturate-induced hypokalemia was 62.2% and the rate of severe hypokalemia was 16.2%. The mean potassium supply per day during thiopentone therapy was statistically significantly different between patients with mild-to-moderate hypokalemic and those with severe hypokalemic (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). Four of 6 patients with severe hypokalemia developed rebound hyperkalemia exceeding 6<span class="elsevierStyleHsp" style=""></span>mEq/L following the cessation of barbiturate infusion. The nadir potassium concentration was 1.5<span class="elsevierStyleHsp" style=""></span>mEq/L and the highest value was 6.8<span class="elsevierStyleHsp" style=""></span>mEq/L. The mean time to reach nadir potassium concentrations was 2.8 days. The mortality rate of the 6 patients was 66.7%. Diecisiete pacientes desarrollaron hipopotasemia leve o moderada (2,6-3,5 mEq/l), y 6 pacientes desarrollaron hipopotasemia grave (<2,5 mEq/l) después de la inducción de la terapia con barbitúricos. La incidencia de hipopotasemia de leve a grave inducida por barbitúricos fue del 62,2% y la tasa de hipopotasemia grave fue del 16,2%. El aporte medio de potasio al día durante el tratamiento con tiopentona fue diferente de forma estadísticamente significativa entre los pacientes con hipopotasemia leve o moderada y entre los que tenían hipopotasemia grave (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001). Cuatro de los 6 pacientes con hipopotasemia grave desarrollaron hiperpotasemia de rebote que superó los 6<span class="elsevierStyleHsp" style=""></span>mEq/l después de la suspensión de la infusión de barbitúricos. Debe evitarse una sustitución excesiva y agresiva del potasio durante el periodo de hipopotasemia inducida por barbitúricos.
