Presentamos el caso de un aneurisma procedente de una rama hemisférica de la arteria cerebelosa superior. La paciente fue una mujer de 68 años con antecedentes de hipertensión arterial e insuficiencia renal de larga evolución. Fue ingresada a causa de una hemorragia subaracnoidea de predominio en fosa posterior y la angiografía demostró un pequeño aneurisma localizado en una rama hemisférica de la arteria cerebelosa superior. En el período preoperatorio, la paciente sufrió un agravamiento de su insuficiencia renal con encefalopatía urémica. La operación fue llevada a cabo el día 40° tras la hemorragia subaracnoidea estando en ese momento asintomática la paciente. El aneurisma fue satisfactoriamente clipado y extirpado usando un abordaje infratentorial supracerebeloso. El postoperatorio fue complicado por hipertensión arterial grave que fue finalmente controlada. El examen anatomopatológico fue compatible con un aneurisma de tipo sacular. El resultado final fue completamente satisfactorio.
We report a case of an aneurysm arising from a hemispheric branch of the superior cerebellar artery. The patient was a 68-year-old woman with long-term hypertension and chronic renal failure. She was admitted because of a subarachnoid hemorrhage mainly in the posterior fossa. An angiogram showed a small aneurysm located on a hemispheric branch of the superior cerebellar artery. During the preoperative period, the renal failure was aggravated and the patient suffered from uremic encephalopathy. The operation was carried out 40 days after subarachnoid hemorrhage when she was asymptomatic. The aneurysm was successfully clipped and extirpated by using an infratentorial supracerebellar approach. The postoperative course was complicated by severe hypertension which was finally controlled. The pathological examination was consistent with a saccular aneurysm. The final outcome was entirely satisfying.
