Resumen

El rasurado o afeitado de la cabeza fue asumido durante muchos años como un procedimiento de rutina que favorece la orientación y la realización de la incisión quirúrgica, considerándose una práctica capaz de prevenir infecciones. Sin embargo, su influencia sobre el riesgo de infección ha sido puesta en duda en muchos trabajos, sin mencionar los favorables efectos psicológicos de los que se benefician los pacientes que no son rasurados. El objetivo del presente trabajo de revisión es comunicar nuestra experiencia, intentando dar respuesta a la pregunta: “¿Está justificado rasurar el cabello antes de la cirugía craneana?”

Realizamos una revisión de 328 pacientes operados sin afeitado del cabello, desde el 1 de Octubre de 1999 hasta el 30 de Septiembre del año 2000. Fueron excluidos los pacientes intervenidos por vía transesfenoidal, los sometidos a drenajes ventriculares externos, los operados para retirar materiales de anteriores cirugías se realizaron para corregir heridas quirúrgicas infectadas. En los pacientes en los que se colocaron derivaciones ventriculoperitoneales, la técnica fue aplicada sólo durante el segundo semestre. Todos los pacientes fueron sometidos al protocolo de antibioterapia que utilizamos regularmente en el Servicio.

La tasa de infección global fue de 1.82%. Se comentan los inconvenientes y las ventajas de la técnica, destacándose el efecto anímico positivo para el paciente.

Summary

Scalp shaving has been a ritual usually said to make easier orientation and surgical access and also to prevent local infection. However, this last aspect has been challenged in several publications and progressive evidence on favourable psychological effects of non-shaved patients has accumulate. The authors review 328 intracranial surgeries performed without scalp shaving from 1 October 1999 to 30 September 2000 in the Department of Neurosurgery of Hospital de S. João. AU patients were included except those undergoing approaches, suchas transphenoidal external ventricular drainages, removal of collocated non-organic materials and surgery for infections. The technique was implemented in patient having CSF shunting only during the second half of the study period. Antibiotic prophylaxis was the same previously used in the Department. The infection rate was 1.82%. The authors comment the disadvantages and benefits of the technique, remarking positive psychological effects for the patient.
