En función de la propia experiencia y de un análisis de la bibliografía, el autor revisa los conceptos actuales de la biomecánica de la instrumentación vertebral con apoyo transpedicular. Se estudian los tres componentes habituales de los implantes: tornillos, barras y conectores, considerando sobre los requerimientos mecánicos de cada uno de ellos. Se comenta la mecánica del ensamblado vértebra-implante en general. Se describen algunos detalles técnicos de la intervención quirúrgica. Sobre estas bases se proponen unos criterios para seleccionar el tipo de implante en cada caso concreto.
Based on his own experience and on the analysis of the related bibliography the autor review the modera concepts on the biomechanics of transpedicular spine instrumentation. The three implant subcomponents are studied: screws, rods and conectors, emphasizing their mechanical implications. The general concepts on mechanics of the vertebral construct are reviewed. Some relevant details of the surgical technique are described. Based on these considerations, the author proposes some criteria to select the type of device that has to be used in each definite patient.
