Resumen

Los autores presentan dos casos de disección carotidea tras un traumatismo sin lesión penetrante en el cuello, que cursaron con un cuadro clínico de deterioro neurológico tras un periodo libre de síntomas, como suele ocurrir en este tipo de lesiones. Los estudios de tomografía computada TC iniciales fueron normales y la angiografía descubrió las lesiones disecantes, una trombosis “en punta de lápiz” de la arteria carótida interna en un caso y una dilatación aneurismática de la carótida interna cervical que embolizó distalmente en la arteria cerebral media del mismo lado en el segundo caso.

Se revisa la etiopatogenia de esta entidad así como las diversas actitudes terapéuticas.

Summary

Two cases of blunt traumatic carotid artery dissection are described, presenting delayed neurological impairment. On both cases initial (CT) were normal. In the first patient angiography showed internal carotid artery obliteration with a “pencil tip” shape and in the second an internal carotid artery aneurysm embolizing in the ipsilateral middle cerebral artery.

The etiopathogenesis and the medical and surgical treatments are reviewed.
