Resumen

Se presenta el caso de un varón de 57 años de edad con fístula espontánea de LCR a través del oído izquierdo secundaria a un defecto en el «tegmen tympani». El paciente presentaba una exudación en la cavidad timpánica izquierda, simulando una otitis serosa. El análisis bioquímico de la serosidad obtenida tras la miringotomía demostró que era LCR. La T.C. mostró la presencia de un defeco en el tegmen. La fístula se cerró con éxito por vía intracraneal a través de una craniotomía temporal. Se expone la patogenia, presentación clínica, diagnóstico y vía de abordaje utilizadas en esta rara entidad.

Summary

The case of a 57-year-old male with spontaneous cerebrospinal fistula through the ear is presented. The patient had fluid in the left tympanic cavity, simulating a serous otitis. Biochemical analysis of fluid obtained following a miringotomy revealed CSF. CT demonstrated a tegmen defect. Fistula was closed successfuily via intracranial approach utilizing a temporal craniotomy. The pathogenesis, clinical presentation, diagnosis and surgical approaches related to this rare entity are discussed.
