Resumen

Se presentan 9 casos de hematoma extradural espinal espontáneo tratados quirúrgicamente entre los años 1980–1994. Siete pacientes eran mujeres. En tres enfermos pudo ser identificado un proceso causal: una malformación vascular en dos de ellos y una alteración de la coagulación en otro. En los restantes no fue posible identificar una causa. Cuatro hematomas eran cervicales, dos cervicodorsales, dos dorsales y otro dorsolumbar. En cuanto a los resultados, 6 pacientes se recuperaron totalmente, 2 de forma parcial y 1 siguió muy incapacitado. Estos resultados están relacionados con la rapidez de la instauración del cuadro neurológico y con el tiempo transcurrido hasta la intervención.

Se revisa la literatura con especial atención a la multiplicidad de factores etiológicos incriminados, formas clínicas de presentación y la importancia de un diagnóstico precoz y tratamiento quirúrgico urgentes.

Summary

The authors report 9 cases of spontaneous spinal epidural hematomas that were operated upon between the years 1980 and 1994. There were 7 women. A causative factor could be demonstrated in three patients: a vascular malformation in two and an hemathological disorder in one. With respect to location, four were cervical, two cervicothoracic, two thoracic and one thoracolumbar. After surgery, six patients were neurologicaly asymptomatic, two had a mild disability and one had asevere disability. Surgical results are related both with the acuteness of onset and the time elapsed to surgery. A review of the literature is performed.
