Lesiones expansivas del lóbulo temporal asociadas a epilepsia en el niño: tratamiento quirúrgico

Resumen

Revisamos 7 niños intervenidos quirúrgicamente, afectos de epilepsia temporal, en los que se identificaron lesiones expansivas del lóbulo temporal. Tres de las lesiones fueron extracerebrales: dos tumores y un quiste aracnoideo. En cuatro pacientes se encontraron lesiones intracerebrales, correspondiendo a tres tumores gliales y a un cavernoma.

Todos los casos fueron investigados mediante TAC y RMN. Las lesiones extracerebrales fueron extirpadas totalmente y el quiste aracnoideo fenestrado. En las intracerebrales se practicó extirpación de la masa y 10bectomía incluyendo el complejo amigdalohipocámpico. En tres de los cuatro pacientes el examen anatomopatológico demostró la existencia de esclerosis mesial concomitante. El seguimiento postoperatorio medio es de más de dos años. Seis se hallan libres de crisis, cuatro de los cuales toman medicación anticomicial. En un paciente las crisis se han reducido en más del 50%.

Summary

A series of seven children presenting with temporal epilepsy operated for temporal expansive lesions is analyzed. Three of the lesions were extraaxial: two of them were tumours, and one an aracnoid cyst. In four patients, we found intraaxiallesions: three glial tumours and one cavernoma. All cases were investigated with CT scan and MRl. The extraaxiallesions were totally removed and the cyst opened. The intraaxial lesions were removed and lobectomy was carried out, including the amigdalohippocampal complexo. In three of the four patients the histopathological examination was compatible with mesial temporal sclerosis. The average postoperatory follow up was more than two years. Six of them have not had any more seizures; four are still on epileptic drugs. In one of the cases the seizures have been reduced in more than 50%.
