Se presenta un caso de hematoma subdural agudo producido por una malformación arteriovenosa occipital paramedial. La presentación clínica inicial fue sugestiva de rotura de aneurisma carotideo. Sin embargo, la tomografía axial computarizada demostró la presencia del hematoma subdural y de la malformación vascular causante. Se comentan la rareza de este modo de presentación clínica y los mecanismos de formación de un hematoma subdural a partir de una malformación vascular sangrante.
A case of large subdural hematoma of the cerebral convexity caused by a paramedian occipital arteriovenous malformation (AVM) is described. The initial clinical presentation was suggestive of a ruptured saccular aneurysm of the internal carotid artery but computed tomography revealed both the subdural hematoma and an occipital intraparenchymal hematoma caused by the ruptured AVM. The rarity and the mechanisms of formation of subdural hematoma from AVMs are discussed.
