Resumen

El término meningioma secretor corresponde a un subtipo de meningioma cuyas características histopatológicas están representadas por un patrón sincitial, presencia difusa de inclusiones hialinas e hiperplasia pericitaria focal. Los hallazgos ultraestructurales e inmunohistoquímicos revelan la máxima expresión de la conocida bipotencialidad, mesenquimal y epitelial, del meningioma. Estas peculiaridades morfológicas se acompañan de una mayor rapidez de presentación clínica y un intenso edema peritumoral, que lo hace difícilmente reconocible como meningioma.

El hallazgo de antígeno carcinoembrionario (CEA) a nivel del tejido tumoral apunta la posibilidad de utilizar este marcador a nivel sérico y de LCR en el seguimiento de estos enfermos, así como incluir al meningioma secretor en el diagnóstico diferencial de pacientes con niveles de CEA elevados.

Summary

Secretory meningioma is the term given to a kind of meningioma characterized histologically by a meningothelial growth pattern, the presence of hyaline inclusions diffusely distributed throughout the tumour and focal pericytic hyperplasia. Ultrastructural and inmunohistochemical findings show the highest occurrence of the well-know bipotential capability of differentiation —epithelial and mesenchymal— of meningiomas. This tumour is often accompanied by cerebral edema and a rapid evolution of symptons, making the clinical diagnosis of meningioma difficult.

The positive staining of neoplastic cells for carcinoembrionic antigen (CEA) suggests the possibility of using this marker not only in the serum but also in the C.S.F. for the follow-up of patients harboring secretory meningioma, and stresses the necessity of including this tumour in the differential diagnosis of patients with a high level of CEA in their sera.
