Paragangliomas are neuroendocrine tumors that usually arise from the adrenal medulla and paraganglias. Location at cauda equina level is extremely infrequent, explaining that they are usually confused with other more frequent tumors at this location like schwannomas and ependymomas. We report two more cases of paraganglioma of the cauda equina, that were studied with magnetic resonance imaging (MRI), in two patients with long lasting low back pain. We review the MRI findings and discuss the therapeutic approach and hystopathological appearance.
