Quistes dermoides de la fontanela anterior

Resumen

Desde 1977 a 1990 seis niños afectados de quistes dermoides de la fontanela anterior han sido estudiados y tratados en esta Unidad de Neurocirugía Pediátrica.

Con la excepción de un niño de 5 años, se trataba de lactantes, de edad comprendida entre 4 y 20 meses y siendo operados 4 de ellos antes del año de edad. Todos presentaban una tumefacción indolora sobre la fontanela anterior, de 2,5 a 6cm de diámetro. La radiografía craneal mostraba afectación ósea en cuatro casos, uno de ellos incluyendo la tabla interna.

La extirpación radical del quiste y su cápsula fue curativa en todos los casos, con un seguimiento entre 5 y 14 años.

El estudio anatomo-patológico estableció el diagnóstico de quiste dermoide, que debe ser diferenciado de los quistes epidermoides, de distinta histología y origen embriológico y raros en esta localización.

Summary

From 1977 to 1991, six children affected by a dermoid cyst of the anterior fontanel were studied and treated in the Unit of Pediatric Neurosurgery.

The age of the patients ranged from 4 months to 5 years and 4 of them were operated before they were 1 year old. All patients presented a painless swelling over the anterior fontanel, the size ranging from 2,5 to 6cm in diameter. Plain X-rays film of the skull showed a bone deffect in four cases, including the inner table in one of them.

Radical excision including whole capsule was curative in all cases, the follow-up ranging from 5 to 14 years.

Pathological examination stablished the diagnosis of dermoid cyst which must be differentiated from epidermoid cyst, because its distinct histology and embriological origin and very rare in this location.
