Se revisan 16 casos de quistes epidermoides intradurales intervenidos en los últimos 12 años. Los síntomas dependieron de la localización, siendo predominantemente las cefaleas en 7 casos y epilepsia en 6 casos. La localización más frecuente encontrada fue a nivel cisternal en 9 pacientes (5 en cisternas del ángulo pontocerebeloso y 4 en cisternas supra y parasellar); 5 pacientes presentaron los quistes localizados en los ventrículos (4 en cuarto ventrículo y uno en ventrículo lateral) y en 2 pacientes se localizaban intraparenquimatosos. El diagnóstico fue principalmente realizado por la tomografía axial computarizada (TAC) seguida de la resonancia magnética (RM). Todos los casos se intervinieron, siendo el resultado excelente en 7, bueno en 6, regular en 1 y en 2 malo, que fueron exitus en reintervenciones de recidivas. El seguimiento de los pacientes se practicó con la TAC y la RM, siendo ésta última de gran valor, diferenciando restos quísticos tumorales de cisternas y/o cavidades quirúrgicas rellenas de líquido cefalorraquídeo.
We review 16 cases of intracranial intradural epidermoid cyst. Symptoms depended on their location, and cephalalgia (7 cases) and epilepsy (6 cases) were the most prominent. The principal location was in the subarachnoid cisterns in 9 cases (5 in the pontocerebellar angle cistern and 4 in te sellar and parasellar cisterns); in 5 patients were located intraventricular, 4 in the fourth ventricle and 1 in the lateral ventricle); in two patients the location was intraparenchymal. Diagnosis was mostly made through computed tomography (CT) and magnetic resonance (MR). All cases were operated and the outcome was excellent in 7 cases, good in 6 cases, fair in 1 and poor in 2 cases. Two patients died after reoperation due to relapses. Follow up was done with CT and MR. MR was useful in differentiating the remains of tumoral cyst from cisterns and/or cerebrospinal fluid filling surgical cavities.
