Resumen

El afeitado del cuero cabelludo para la práctica de intervenciones neuroquirúrgicas es un tema actualmente en discusión.

Hemos comparado la incidencia de infección con el afeitado de la zona de la incisión con la registrada recurriendo al afeitado completo habitual. Para ello, hemos considerado prospectivamente los procedimientos craneales realizados entre enero y diciembre de 1993, en nuestro Servicio.

Se han excluído las cirugías de urgencia, los procedimientos con implantación de material extraño, sobre heridas supuradas y aquellos no autorizados para su inclusión en el estudio por el cirujano o el paciente.

Se han dividido aleatoriamente los pacientes en dos grupos: treinta y tres fueron afeitados sólo en la línea de incisión y veinticinco fueron completamente afeitados. Otros treinta y siete pacientes más fueron seleccionados al azar entre aquellos operados en el mismo período de tiempo y fuera del protocolo por diferentes razones.

Hemos encontrado tres infecciones de herida en el grupo de afeitado mínimo, ninguna entre los afeitados completamente y dos pacientes en el grupo no incluído en el protocolo. Estas diferencias no alcanzaron significación estadística.

Comentamos estos resultados e indicamos a qué pacientes se les puede ofrecer el método de afeitado mínimo.

Summary

Scalp shaving in Neurosurgery is now a subject in discussion. We have compared the incidence of postoperative infection with the shaving of the incision area alone versus the conventional full shaving of the scalp used in our Department.

For this purpose, we prospectivelly considered the cranial procedures made between january and december of 1993. We excluded emergency surgery, implantation of foreing material procedures, surgery of suppurated wounds and those unauthorized to be included in the study by the surgeon or by the patient.

We have randomly divided the patients in two groups; thirty-three patients were shaved only in the incision line and twenty-five had all the scalp shaved. Thirty-seven more patients were randomly selected among those operated in the same period of time out of protocol for different reasons.

We found three wound infections in the minimal shaving group, none in the those completely shaved group, and two patients in the group out of protocol. These differences did not reach statistical significance.

On the basis of our results we indicate the patients to whom minimal shaving method could be offered.
