Una herniación discal a través de la duramadre es extraordinariamente rara. En el presente artículo se describe un caso de extrusión discal masiva transdural, originando un cuadro agudo de compresión de cola de caballo. La experiencia con dicho caso y la revisión de la literatura sugiere que un antecedente de cirugía previa en la zona puede condicionar adherencias entre la duramadre y el ligamento vertebral común posterior, facilitando que, en el caso de ocurrir una hernia discal, ésta penetre a través de la duramadre hacia el espacio subaracnoideo.

Our present experience and a review of the literature suggest that adhesions between the dura mater and the posterior longitudinal ligament, fixing the dUI:-al sac to the dorsal surface of the vertebral bodies, contribute to the possi· bility of a transdural disc herniation.
