A 22-year-old sportsman patient with a synovial cyst in the axilla of S1 root which was attached to the ventrolateral aspect of the dura mater, is presented. This is the first reported case of spinal synovial cyst in this anatomic location, and it supports previous evidences of synovial cysts that were not related to the joint facets.
