Hemos seleccionado 50 pacientes afectos de hernia discal no emigrada, mediante estudios radiológicos y exploración clínica, con criterios rigurosos: Lasegue a menos de 70 grados y datos positivos neurorradiológicoso Han sido intervenidos mediante nuclectomía automatizada percutánea con resultados altamente satisfactorios. Cuatro pacientes son considerados como malos resultados. Seis regulares y 40 buenos. No hemos apreciado complicaciones.
La cantidad de material discal extraído fue como media 2’4 gramos, sin que esa cantidad mayor o menor influyese en el resultado, lo que parece indicar que la mejoría se debe a descompresión de la raíz, más que a extirpación total del disco.
Consideramos pues la técnica útil para casos concretos, sin excluir en otros pacientes otras técnicas.
Filty patients with non-extruded lumbar disc herniation were selected for percutaneous automated nucleotomy on strict clinical and radiological criteria. Overall, the results were highly satisfactory. Only four patients had poor result, six were moderately improved and forty experienced a satisfactory resulto There were no complications.
The amount of disc material removed averaged 2.4 grams, but it did not correlated with the clinical outcome of the patient, suggesting that improvement is related to root decompression, more than to complete disc extirpation. Thus, we consider percutaneous automated nucleotomy very useful for selected cases, not excluding the use of alternative conventional techniques in other patients.
