Conservative therapy has proven effective, demonstrating that adequate immobilization of the injured spinal segment can achieve fracture consolidation and potentially reduce spinal canal stenosis through the resorption of bony fragments.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> However, when conservative therapy is not feasible, internal stabilization with metal fixators is considered the preferred treatment.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Surgical strategy for TLBF is straightforward. Anterolateral approaches are particularly effective for anterior decompression and are recommended for patients with neurological deficit or exhibiting severe kyphotic deformities.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> According to a meta-analysis by Hinojosa-Gonzalez et al.,<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> both anterolateral and posterior surgical approaches are equally effective in managing TLBF. These methods show a significant reduction in operation time, blood loss, and length of hospital stay, particularly when employing standard pedicle screw fixation (PSF).</p><p id="par0015" class="elsevierStylePara elsevierViewall">Despite the availability of numerous prospective studies, systematic reviews, and meta-analyses discussing PSF methods, a key limitation remains significant heterogeneity in patient groups, including individuals with both spinal cord trauma and those with distraction or translational injuries. Currently, no systematic studies specifically address the optimal PSF method for patients with TLBF without neurological deficit.</p><p id="par0020" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Objective:</span> To conduct a systematic review of studies on various PSF methods used for treating neurologically intact TLBF and to identify the most effective and safe approaches.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect1060">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Study selection</span><p id="par0025" class="elsevierStylePara elsevierViewall">Systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> The study was registered in the PROSPERO (№ CRD42024531093).</p><p id="par0030" class="elsevierStylePara elsevierViewall">The search strategy used for the PubMed database included the following keywords: (Lumbar vertebrae [MeSH] OR Thoracic vertebrae [MeSH] OR spine [MeSH] OR Thoracolumbar [TIAB] OR thoraco-lumbar [TIAB] OR thoraco lumbar [TIAB] OR burst [Title]) AND (Injur* [TIAB] OR trauma* [TIAB] OR fractur* [TIAB] OR dislocation* [TIAB]) NOT animal [MeSH] NOT comment [PT] NOT letter [PT] NOT editorial [PT] NOT news [PT] NOT “newspaper article” [PT] NOT osteoporosis [MH] NOT osteoporotic fractures [MH] NOT osteoporo* [TITLE] NOT spinal neoplasms [MH] NOT tumor* [TITLE] NOT malignan* [TITLE].</p><p id="par0035" class="elsevierStylePara elsevierViewall">The inclusion criteria were as follows: (1) articles published between January 1, 2004, and December 31, 2023; (2) full-text availability in English; (3) fractures classified as type A3 or A4 according to the AOSpine classification, or types A, B, or C burst fractures as per the Denis classification, or instances where the author explicitly identifies the presence of a "burst" fracture, irrespective of classification; (4) no spinal cord or nerve root injuries present at the time of patient admission; (5) patient age of 18 years or older; (6) articles must include descriptions of treatment outcomes or any complications developed in the study: (7) a minimum mean follow-up period of 12 months. Articles failing to meet these criteria were excluded from the systematic review.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Two reviewers (authors 6 and 7) independently applied the eligibility criteria and selected studies for inclusion in the systematic review. Disagreements between their judgments were resolved by a third reviewer (author 5). The article searches and selection process is illustrated in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Data collection</span><p id="par0045" class="elsevierStylePara elsevierViewall">Data extraction was conducted by two participants (authors 2 and 3), with any disagreements between their judgments resolved by a third reviewer (author 4).</p><p id="par0050" class="elsevierStylePara elsevierViewall">The data from each article were recorded in the corresponding cells of the table. Basic information included sample size, average patient age, gender distribution, diagnosis, and mechanism of injury. The primary data block included details on the PSF techniques (percutaneous, midline, or paramedian approaches), the number of screws used, the fusion method, timing of implant removal, and radiological indicators at admission, post-surgery, and during the final follow-up. Additional data included the length of hospital stay, average duration of final follow-up, severity of pain using the Visual Analogue Scale (VAS), and quality of life at final follow-up as measured by the Oswestry Disability Index (ODI).</p><p id="par0055" class="elsevierStylePara elsevierViewall">For radiological assessments, recorded parameters included the degree of kyphotic deformation of the affected segment using the Cobb angle, the percentage of compression of the anterior vertebral body (AVBCP) relative to unaffected segments, and the degree of spinal canal stenosis determined by the mid-sagittal diameter.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">Statistical analysis was performed using the PC STATISTICA software (Version 10) (StatSoft@ Inc., USA). The normality of data distribution was assessed using the Shapiro-Wilk test. Comparison of continuous data with non-normal distribution was conducted using the Mann–Whitney U test or the Kruskal–Wallis test. Statistical hypothesis testing was performed at a critical significance level of p = 0.05.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Article selection</span><p id="par0065" class="elsevierStylePara elsevierViewall">An initial search in the PubMed database yielded 1255 articles. After filtering for age and language, the abstracts were screened, resulting in 189 studies being selected for full-text review. Of these, 69 articles <a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6–74</span></a> met the inclusion criteria and were included in the systematic review (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), presenting treatment data for a total of 3248 patients.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Authors commonly divided their samples into two or more groups based on the specifics of the surgical interventions. Altogether, the 69 articles reported on 116 groups of patients with neurologically intact TLBF. Specific injury levels were detailed for 99 of these groups, with 69 describing injuries at the Th10-L2 or Th11-L2 levels, 27 involving the thoracolumbar junction and the entire lumbar spine, and 3 focusing solely on the L1-L4 vertebrae. In 17 groups, the injury level was broadly categorized as the thoracolumbar junction without specifying vertebrae.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Gender distribution was documented in 64 articles, representing 2917 patients with a composition of 1877 males (64.3%) and 1040 females (35.7%). The median value for average age across all studies was 42 years. The mechanism of injury was specified in 37 studies involving 1877 patients, with falls from height (47.8%) and road traffic accidents (39.4%) being the most common causes of TLBF. The characteristics of the studies are detailed in Tables A.1 and A.2.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Analysis of PSF extension and fusion application</span><p id="par0080" class="elsevierStylePara elsevierViewall">The extent of PSF was analyzed across 116 patient groups. Most of patients (95 groups, 2774 individuals) received short-segment PSF, immobilizing two adjacent motion segments. In 15 groups (334 patients), PSF extended over three or more segments, while monosegmental PSF was performed in 6 groups (140 patients).</p><p id="par0085" class="elsevierStylePara elsevierViewall">For posterior fusion, two techniques were primarily employed. Posterior lateral fusion (PLF), involving decortication of the posterior elements and placement of bone grafts, was performed in 48 groups (1231 individuals). The transforaminal lumbar interbody fusion (TLIF) technique was used in 2 groups (37 patients). A total of 66 groups (1980 patients) underwent PSF without fusion.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The duration of PSF retention was recorded for 98 groups. Among them, PSF was removed, on average, 12 months post-surgery in 30 groups (894 patients). In 68 groups (1786 patients), PSF was maintained throughout the entire follow-up period.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Thus, data for 95 patient samples (2680 individuals) detailed the extent of fixation, fusion application, and whether PSF was removed or retained, forming five comparison groups. Statistical analysis revealed significant advantages of short-segment fixation in terms of shorter operation times and reduced intraoperative blood loss (p = 0.001 and p < 0.001, respectively). A higher incidence of deep surgical site infections was significantly associated with extensive fusion compared to other methods of PSF (p = 0.031). No significant differences were observed in other measured outcomes (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Analysis of PSF methods</span><p id="par0100" class="elsevierStylePara elsevierViewall">Short-segment PSF without fusion was performed using three types of approaches: midline (23 groups, 643 patients), paramedian (8 groups, 323 patients), and percutaneous (33 groups, 921 patients). The findings indicated that percutaneous PSF, typically performed on patients presenting with lower AVBCR and milder kyphotic deformities, had less substantial correction potential. However, it significantly reduced blood loss, shortened operation duration, and decreased the length of hospital stay (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Intermediate screw application</span><p id="par0105" class="elsevierStylePara elsevierViewall">Within the cohort of 91 patient groups treated with short-segment PSF, the application of intermediate screws at the fractured vertebra level was specifically documented. Intermediate screws were utilized in 41 groups, encompassing 1113 patients. Conversely, a standard four-screw PSF configuration was implemented in 53 groups, involving 1550 patients. Given the earlier noted limitations of minimally invasive techniques, the efficacy of intermediate screws was separately assessed for patients treated with percutaneous fixation and those undergoing PSF via a posterior midline approach. Statistical analyses conducted (<a class="elsevierStyleCrossRefs" href="#tbl0015">Tables 3 and 4</a>) failed to demonstrate significant benefits from the addition of intermediate screws in these short-segment PSF procedures. Due to the limited number of groups, patients with PSF via paramedian approaches were not included in the statistical comparison. In two articles the surgical approach was not specified.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Decompression in patients with neurologically intact TLBF</span><p id="par0110" class="elsevierStylePara elsevierViewall">Among the 116 patient groups studied, laminotomy or laminectomy for the removal or repositioning of fragments was performed in only 3 groups. For most patients upon admission, the median value for average spinal canal compression rate was 38.65% [Interquartile Range (IQR), 32.1–47.0]. By the time of the final assessment, this rate had significantly decreased, nearly halving to 20.8% [IQR, 14.7–23.7].</p></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0115" class="elsevierStylePara elsevierViewall">Despite numerous studies addressing surgical interventions for spinal fractures, the treatment of TLBF in neurologically intact patients remains controversial. While the surgical strategy for patients with neurological deficits is widely recognized and involves decompression, restoration of spinal column alignment, and fusion of the injured segment.<a class="elsevierStyleCrossRef" href="#bib0375"><span class="elsevierStyleSup">75</span></a> the approach for patients without neurological deficits is not straightforward. The existing literature does not offer a consensus on this issue, highlighting a significant gap in the evidence. A systematic review or meta-analysis represents the most robust method for improving the quality of evidence by fusing results from various studies. In the context of TLBF, 12 systematic reviews and meta-analyses that explore different aspects of PSF could be identified.<a class="elsevierStyleCrossRefs" href="#bib0380"><span class="elsevierStyleSup">76–87</span></a> However, these studies frequently include patients with neurological impairments, who typically present with more severe kyphotic deformities, greater vertebral body compression, and more severe spinal canal stenosis. These factors contribute to higher complication rates and poorer outcomes at final follow-up. Furthermore, these meta-analyses typically include only 5–6 comparative studies, a limitation that can substantially affect the reliability of their conclusions and lead to inconsistent results. In conducting this systematic review, we found no studies specifically addressing the optimal surgical treatment for neurologically intact patients with TLBF.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Our study primarily focused on the extent of posterior spinal fusion (PSF) and the necessity of conducting bone fusion via the posterior approach. According to Ituarte et al.,<a class="elsevierStyleCrossRef" href="#bib0430"><span class="elsevierStyleSup">86</span></a> more extensive PSF led to less progression of postoperative kyphosis and decreased the incidence of implant-related complications. Concerning the necessity of fusion, three meta-analyses have been published.<a class="elsevierStyleCrossRefs" href="#bib0405"><span class="elsevierStyleSup">81–83</span></a> The most recent analysis<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">82</span></a> indicated, based on comparisons involving only six patient groups, that posterior lumbar fusion (PLF) offers no long-term advantages. Present study revealed that PLF significantly extends the duration of the surgery and heightens the risk of infectious complications in cases of prolonged PSF. Conversely, based on the obtained data, it does not significantly impact the correction of kyphotic deformity or the restoration of vertebral height in fractured vertebrae in patients with neurologically intact TLBF.</p><p id="par0125" class="elsevierStylePara elsevierViewall">The question of temporary PSF was investigated in two meta-analyses.<a class="elsevierStyleCrossRefs" href="#bib0395"><span class="elsevierStyleSup">79,85</span></a> Both authors concluded that removing the fixation system leads to improved quality of life in the postoperative period without subsequent deterioration of radiological indicators. It should be noted that these conclusions could also be influenced by the number of included studies and data processing methodology. For instance, Kweh et al.<a class="elsevierStyleCrossRef" href="#bib0395"><span class="elsevierStyleSup">79</span></a> used only two studies to construct forest plots for the VAS and ODI, while Visagan et al.<a class="elsevierStyleCrossRef" href="#bib0425"><span class="elsevierStyleSup">85</span></a> solely relied on descriptive statistical methods. In the present systematic review, when comparing temporary and permanent short-segment PSF, we used results from 27 and 20 articles, respectively. Authors of the included studies justified the removal of PSF systems by the need to prevent implant breakage or pool-out. However, no statistically significant differences were found in the frequency of implant-related complications, loss of achieved kyphosis correction, or quality of life at the time of final assessment according to the VAS and ODI between groups with permanent and temporary short-segment PSF (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0130" class="elsevierStylePara elsevierViewall">The choice of surgical approach is crucial in managing thoracolumbar burst fractures. Four published comparative meta-analyses highlight the benefits of percutaneous fixation, which include reductions in operation time, blood loss, and postoperative hospital stays, as well as a decrease in the frequency of infectious complications.<a class="elsevierStyleCrossRefs" href="#bib0400"><span class="elsevierStyleSup">80,83,84,87</span></a> Additionally, this technique is credited with better correction of kyphotic deformity and enhanced quality of life at the final follow-up.<a class="elsevierStyleCrossRefs" href="#bib0400"><span class="elsevierStyleSup">80,83,87</span></a> In the present systematic review, short-segment percutaneous fixation was found to significantly reduce intraoperative blood loss, operation duration, and average hospital stay compared to the midline approach (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). However, it is important to note that percutaneous fixation was applied to patients with less severe kyphotic deformities and vertebral body compression, indicating a less severe injury profile. Furthermore, data obtained suggest that the degree of kyphotic deformity correction with the percutaneous method was significantly lower than that achieved with the open midline approach. No significant differences were observed in VAS and ODI scores at the final follow-up, nor in the frequency of infectious or implant-related complications between the minimally invasive and open posterior spinal fixation groups. These findings indicate potentially limited repositioning capabilities of percutaneous fixation using polyaxial screws, as commonly employed by many authors. Although some studies have explored the use of monoaxial screws and specialized fracture reduction systems, but their number was too low to allow for a statistically significant comparison.</p><p id="par0135" class="elsevierStylePara elsevierViewall">The debate surrounding the use of intermediate screws at the fracture level is highlighted in two meta-analyses.<a class="elsevierStyleCrossRefs" href="#bib0405"><span class="elsevierStyleSup">81,82</span></a> Both studies reported an improvement in the degree of kyphotic deformity correction and an increase in fractured vertebra height, alongside a reduction in the frequency of implant-related complications for patients receiving additional screws. Notably, these meta-analyses included patients with not only spinal cord injuries but also translational and distraction injuries (AOSpine types B and C). Comprehensive literature search found no specific conclusions or recommendations regarding the use of intermediate screws in patients with neurologically intact TLBF. In the present systematic review, the addition of screws did not have a statistically significant impact on any investigated parameters, potentially due to the exclusion of patients with distraction or translational injuries, whose correction presents more complex challenges.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The necessity of laminectomy or laminotomy in patients with TLBF remains unaddressed by any systematic review or meta-analysis. Most authors opted against decompression even when spinal canal stenosis reached 50%. Surgical intervention in the spinal canal was limited to only two studies, which involved the removal or repositioning of bone fragments. Instead, simple internal immobilization of the injured segment generally sufficed to achieve favorable radiological outcomes. Patients showed lysis of bone fragments and a reduction in spinal canal compression by nearly half, mirroring the results of conservative treatment for TLBF.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> This reinforces the argument against the necessity for spinal canal decompression in such cases.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Study limitations</span><p id="par0145" class="elsevierStylePara elsevierViewall">The primary limitations of this study stem from its design and the heterogeneity of the included patient samples. Surgeries were performed by different surgeons across various regions, each possibly possessing different levels of expertise and working in different operating environments. Due to the large number of articles, cohorts, and investigated parameters, we refrained from using the traditional Newcastle-Ottawa Scale for quality assessment of the included studies. Publication bias could have been adequately addressed using forest plots, Egger’s test, or Begg’s test. However, the lack of published comparative studies specifically on neurologically intact fractures precluded such analysis.</p><p id="par0150" class="elsevierStylePara elsevierViewall">We also noted the consolidation of most included injuries into a single mixed category of A3 and A4 fractures according to the AOSpine classification, as well as types A, B, and C injuries according to the Denis classification. Distinguishing thoracolumbar burst fractures (TLBF) into more specific subtypes would have facilitated a more detailed analysis of the outcomes.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Despite these limitations, the present study consolidates the entire published experience regarding neurologically intact thoracolumbar burst fractures. We believe that the data obtained may assist surgeons in selecting the most appropriate surgical treatment method, thereby avoiding ineffective procedures and improving both short-term and long-term outcomes.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Conclusion</span><p id="par0160" class="elsevierStylePara elsevierViewall">Short-segment pedicle screw fixation without decompression could be the optimal surgical treatment method for neurologically intact thoracolumbar burst fractures. The use of posterior lateral fusion in this context may increase the deep surgical site infection rate without reducing the frequency of implant-related complications or improving long-term treatment outcomes. The percutaneous approach remains the preferred technique; however, its limited reduction capabilities should be carefully considered during surgical planning for patients with severe kyphotic deformities. The application of intermediate screws in such patients has not demonstrated significant advantages. Removing the fixation system has not led to a significant decrease in implant-related complications or improvement in quality of life. Future comparative studies of various spinal fixation techniques, supported by a high level of evidence, are necessary to establish robust treatment recommendations and corresponding protocols.</p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Funding</span><p id="par0170" class="elsevierStylePara elsevierViewall">No special funding was received in support of this study. The inclusion criteria were: (1) publication dates from January 1, 2004, to December 31, 2023; (2) availability of full-text articles in English; (3) thoracolumbar burst fractures without neurological deficits; (4) patients aged over 18; (5) reports on treatment outcomes or complications; (6) a mean follow-up period of at least 12 months.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">A total of 69 articles covering 116 patient groups were included. Our analysis highlighted the advantages of short-segment fixation without fusion over monosegmental, short-segment and long-segment fusion in terms of shorter operation times and reduced intraoperative blood loss (p = 0.001 and p < 0.001, respectively). Extensive fusion was associated with a significantly higher frequency of deep surgical site infections compared to other PSF methods (p = 0.043). Percutaneous pedicle screw fixation, applied to patients with lower body compression rates and kyphotic deformities, led to less potential for correction (p = 0.004), yet significantly decreased blood loss (p = 0.011), operation duration (p < 0.0001), and hospitalization duration (p < 0.0001).
