Introduction: Coil embolization is an important technique to treat intracranial aneurysms. Complications of intraprocedural coil migration have been reported several times in the literature. However, delayed coil migration is extremely rare.

Objectives: To describe delayed migration of a coil loop after embolization of a petrous segment of the internal carotid artery pseudo aneurysm.

Case report: A 62-year-old woman with an incidental pseudoaneurysm of the petrous segment of the internal carotid artery underwent successful coil embolization. Several months after the procedure, the patient returned with the foreign body sensation in the ear. CT scan imaging demonstrated a migration of a loop coil until externalization by external auditory canal. Angiography showed exclusion of the pseudoaneurysm with severe stenosis of the internal carotid artery but with adequate filling of the cerebral hemisphere via the posterior communicating artery.

Discussion: Delayed migration of a coil loop after embolization is rare and to our knowledge this is the first case reported of this kind.
