Los aneurismas son la patología vascular intracraneal más frecuentemente asociada a una arteria trigeminal primitiva persistente (ATPP). Sin embargo los originados de ésta o del segmento intracavernoso de la arteria carótida interna son extremadamente infrecuentes y sus características angiográficas condicionan la opción terapéutica a elegir. Presentamos el caso de una paciente con un aneurisma intracavernoso asociado a ATPP con flujo en sentido carótido-basilar e inversión del flujo con relleno del aneurisma en la angiografía vertebral con compresión de la carótida interna ipsilateral. Se eligió la embolización como tratamiento óptimo. Se revisa la bibliografía y se comentan las divrsas posibilidades terapéuticas en base a criterios hemodinámicos.
Although aneurysms are the most commonly intracranial pathology associated with persistent primitive trigeminal artery (PTA), those of the PTA itself or the intracavernous segment of the internal carotid artery are extremely rare. We report a case of intracavernous aneurysm associated to a persistent PTA and discuss its angiographic features. We considered embolization as the best therapeutic choice. We discuss the different therapies and their indications based on hemodynamic criteria.
