Hidrocefalia en los tumores de fosa posterior en la infancia. ¿Necesidad de derivación permanente de líquido cefalorraquídeo?

Resumen

Por su frecuencia los tumores cerebrales, especialmente los localizados en la fosa posterior, ocupan un lugar importante dentro de la oncología pediátrica.

Se revisan 67 casos de tumores infratentoriales. El diagnóstico anatomopatológico fue de meduloblastoma en 31 casos (46,2%), astrocitoma en 25 casos (37,3%), ependimoma en 6 casos (8,9%) y papiloma de plexos coroideos en 2 casos. En el resto de los pacientes se encontraron otro tipo de tumores infrecuentes en esta localización.

El 89,5% de los pacientes tenían hidrocefalia en el momento de efectuar el diagnóstico, pero sólo el 40,2% requirieron una derivación de LCR permanente.

Nuestros resultados indican que la implantación de un sistema derivativo de L.C.R. puede ser evitada cuando es posible extirpar la tumoración de modo precoz y radical.

Summary

The authours analyze the clinicopathological findings in 67 patients with posterior fossa neoplasms treated during the last seven years.

Histological diagnoses were: Meduloblastoma (31 cases), astrocytoma (25 cases), ependymoma (6 cases), papilloma of the fourth ventricle (2 cases) and miscelIaneous lesions in the remaining cases.

The majority of the patients (89,5%) presented with hydrocephalus but only 40,2% required permanent CSF shunting. The correlation between the type of tumor, the timing of surgery and the need for permanent shunting is analyzed. Our experience indicates that when the tumor is removed early in the course and radically, CSF shunting can be usually avoided.
