Resumen

El objeto del presente estudio es presentar la experiencia de los autores en el tratamiento endoscópico de los quistes coloides del tercer ventrículo. Seis pacientes fueron intervenidos en estos últimos 3 años en nuestro servicio de quistes coloides, cinco de ellos en el tercio anterior del III ventrículo, y otro en situación atípica. La mayor parte de los casos presentaron hipertensión intracraneal como síntoma inicial. En cinco de los pacientes se consiguió extirpar el quiste de forma completa mediante técnicas endoscópicas. En un caso se transformó el abordaje endoscópico en microquirúrgico por sangrado. Los tiempos operatorios fueron menores que los habituales en microcirugía de lesiones intraventriculares. Cinco de los pacientes están actualmente asintomáticos, presentando nuestro primer caso alteraciones de memoria permanentes. Se concluye que la exéresis endoscópica es en la actualidad una alternativa segura, menos invasiva que la microcirugía tradicional y con buenos resultados.

Summary

This article reviews the experience of the authors in the endoscopic management of colloid cysts of the third ventricle. Since 1996 six patients were treated of colloid cysts of the third ventricle in our department, five of them were placed in the anterior third of the third ventricle and another occupied a more posterior location. Most of the patiens presented with intracranial hypertensin. In five cases we achieved a total removal of the cyst with endoscopic techniques, and in one case we transformed the procedure in a classical microsurgical approach because of bleeding. The duration of the surgery was signifficantly shorter than the standard transcortical or trascallosal microsurgery. Five of the patients are free of symptoms in the follow-up, showing the first patient permanent memory disturbances. We conclude that endoscopic removal is a safe and less invasive alternative to microsurgery in the surgical management of colloid cysts, with good results.
