O autor revê o valor da histopatologia e da cinética das células tumorais no prognóstico dos tumores gliais. Aspectos microscópicos como sejam a infiltração dos tecidos vizinhos ou para lá dos confins do tumor e a constatado de multifocalidade tumoral são factores importantes na recorrência dos gliomas objecto de recessóes macroscópicas totais. As classificações histopatológicas destes tumores associadas a tentativa de correlac,ao clínica sao revistas criticamente. É realçada a importância da biópsia estereotáxica no diagnóstico histológico e na opção terapêutica dos gliomas. Os métodos de avaliação prática da capacidade proliferativa das células tumorais são a determinação dos índices proliferativos do PCNA, Ki-67 e MIB1, que se correlacionam com o comportamento biológico dos gliomas.
The author reviews the value of the histology and tumor cell kinetics in the prognostic of gliomas. Microscopic features like infiltration of perilesional tissue or beyond the immediate confines of the growth, and occurrence of multifocality are important factors to tumor's recurrences after macroscopically total resections. Histopathological classifications of gliomas attempting a correlation with their prognostic are critically reviewed. The importance of stereotactic biopsy in the histological diagnosis and therapeutic approach of gliomas is stressed. The routine methods of evaluation of the proliferative capacity of tumoral cells are the PCNA, Ki-67 and MIB1 index labellings, which correlate with the clinical behaviour of gliomas.
