Image shows the protrusion of the optic nerve head in both retinas, a radiological sign of papilloedema and IH (black arrows). k) ICP waveform showed persistently elevated values, with a mean of 28 mmHg.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Arteriovenous malformations (AVMs) cause circulatory and functional disturbances in adjacent and remote areas of the brain<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> and are classified as pial, dural (dural fistulas) and mixed depending on the characteristics of their arterial supply. Pial AVMs (pAVMs) are most often identified in the context of diagnostic tests for cerebral haemorrhage, seizure or chronic headache.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Intracranial hypertension (IH), although considered reasonably prevalent in patients with unruptured, non-hydrocephalus-causing pAVMs, has been described as an initial clinical manifestation in a small number of cases in the literature.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> In an updated review on this issue, only 19 papers could be identified (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Of these, only one case underwent endovascular treatment alone with Onyx®.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> IH was generally resolved after surgical, endovascular or combined treatment of the pAVM (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Case report</span><p id="par0010" class="elsevierStylePara elsevierViewall">We present the case of a 36-year-old male, with no relevant medical history, referred to the accident and emergency department from the ophthalmology outpatients for bilateral papilloedema. A non-contrast and contrast-enhanced brain CT scan confirmed a probable right occipital pAVM, with no evidence of hydrocephalus or associated acute bleeding. Non-urgent brain magnetic resonance imaging (MRI) showed a right occipital pAVM consisting of a nidus measuring 33 mm × 22 mm × 20 mm, arterial supply from the right posterior cerebral artery and venous drainage to the superior sagittal sinus (SSS). An associated right transverse venous sinus stenosis was also identified. Arteriography confirmed pial arterial inflow not only from the right posterior cerebral artery, but also from the right middle cerebral artery (MCA). The pAVM was categorised as grade II according to the Spetzler-Martin grading system. It was then decided to perform an intracranial pressure (ICP) study using an intraparenchymal fibre optic system and monitor (Camino®, Natus, Middleton, Wisconsin, USA) and data reading and analysis software (Labchart®, AD Instruments, Dunedin, Otago, New Zealand). ICP was continuously monitored for 48 h, obtaining mean values of 28 mmHg, thereby confirming IH (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). An interventional therapeutic procedure was performed under general anaesthetic. Selective catheterisation of the pAVM feeding arteries was performed. Initially, using the <span class="elsevierStyleItalic">pressure cooker</span> technique, the MCA-dependent branches were embolised with Onyx®. Next, once part of the nidus was excluded, the arterial supply dependent on the right posterior cerebral artery was occluded using the same technique and embolisation material. The final arteriographic series showed satisfactory and complete exclusion of the pAVM. The immediate postoperative period was uneventful. Back on the hospital ward, a new ICP study was performed using the same system and study regimen as described above. Mean values of 12 mmHg were obtained, with resolution of IH signs and symptoms. The patient was discharged home a few days later. Bilateral papilloedema progressively subsided. Successive regular follow-up imaging studies since then have shown complete exclusion of the pAVM at all times (most recently an arteriographic follow-up five years after the procedure) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Discussion</span><p id="par0015" class="elsevierStylePara elsevierViewall">Pial AVMs consist of one or more dysplastic arteries (afferent or feeding) and veins (efferent or draining) which converge in a vascular nidus, in the absence of intervening normal brain parenchyma, causing abnormal blood flow within it.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The estimated annual incidence of pAVMs is between 1.10 and 1.42 cases per 100,000 population. The prevalence, according to post-mortem studies, ranges widely from five to 613 cases per 100,000 population.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> Cerebral haemorrhage is the most common form of presentation of these lesions, manifesting in 38%–65% of cases.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> The risk of bleeding from an unruptured and untreated pAVM ranges from 1% to 3% per year, and up to 5% in subjects who have bled on some occasion. As such, previous bleeding is the main risk factor for rebleeding.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> Seizures are the second most common form of initial presentation, occurring in 15%–35% of cases. Between 5% and 15% of cases initially present with a focal neurological deficit (not associated with simultaneous haemorrhage) or non-specific headache.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> IH syndrome (headache, papilloedema), as the initial clinical presentation of an unruptured, non-hydrocephalus-causing pAVM has been reported in a small number of cases in the literature.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">In the literature review conducted for this paper, a total of 19 cases of unruptured pAVMs were identified whose first clinical signs were IH, in the absence of previous hydrocephalus or haemorrhage. No clear gender predilection was identified (10 males and nine females). The mean age of the patients was 24. The right cerebral hemisphere was most commonly affected (13 cases, 68.42%). The most frequent specific location was right parieto-occipital region (four cases, 21.05%), followed by the right parietal region (three cases, 15.78%). The most commonly involved arterial vessel was the right MCA (10 cases, 52.63%). The most commonly involved venous drainage structure was the SSS (13 cases, 68.42%). Concomitant structural venous sinus abnormalities were identified on three occasions (three right transverse sinus stenoses and one left transverse sinus stenosis). Resective surgery alone was the most commonly used therapeutic procedure (eight cases, 42.10%). The combination of embolisation and resective surgery was used in three cases (15.78%). All resective procedures were documented as complete. Embolisation alone was used in three cases (15.78%). Of these, two were partial and one complete. Only one case was specifically treated with Onyx® on its own. Overall, the outcome of IH was favourable in all patients who underwent resective surgery alone, embolisation combined with resective surgery and embolisation alone. However, post-procedural neurological complications were more common in the first two groups (mainly hemiparesis and homonymous hemianopia). No complications inherent to the procedure were documented in patients treated by embolisation alone (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0030" class="elsevierStylePara elsevierViewall">The traditional view that AVMs are a congenital condition which is static over time is being replaced by a <span class="elsevierStyleItalic">dynamic</span> perspective, which holds that these lesions are able to change and develop over a patient's lifetime.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Although the exact origin is not fully understood, there is strong evidence to suggest that AVMs arise in relation to the concomitant existence of structural abnormalities of the venous sinuses leading to the development of local venous hypertension. This would lead to the development of regional tissue hypoxia, secreting angiogenic factors (VEGF, TGF-B) which would initiate the cascade of vascular injury. Structural abnormalities such as SSS aplasia<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> or venous sinus thrombosis<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> have been widely documented in the scientific literature supporting this theory. Our case had a clear stenosis of the right transverse sinus and, although we cannot confirm a clear causal relationship with the development of pAVM, we have to question whether or not it might be involved in the process.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The reason why IH develops in patients with an unruptured or non-hydrocephalus-causing pAVM is still not fully understood.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> However, two likely pathophysiological factors have been described in the literature: 1) increased intracranial blood volume due to venous hypertension caused by flow overload in the distal region of the pAVM and the consequent decrease in venous return from the surrounding brain areas; and 2) increased intracranial cerebrospinal fluid volume caused by reduced reabsorptive capacity.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> It seems reasonable to assume that IH is not exceptional in patients with an unruptured, non-hydrocephalus-causing pAVM, but rather that its development is part of a certain intrinsic pathophysiological mechanism. <span class="elsevierStyleItalic">Hydraulic theory</span> takes the above elements into consideration and provides a comprehensive framework for understanding the haemodynamics of pAVMs and their effect on cerebrospinal fluid dynamics. According to this theory, the degree of IH caused by the pAVM will depend, among other factors, on the type of venous drainage of the pAVM (number of veins, presence of anastomoses) and whether or not the maximum distensibility of the pAVM is reached. This theory also provides a model for understanding the occurrence of local mass effect and oedema surrounding unruptured pAVMs and the development of hydrocephalus in patients with unruptured pAVMs. The theory also contributes to a better appreciation of the pathophysiology of dural fistulae, vein of Galen malformation and disorders related to autoregulation in patients with AVMs.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">Endovascular treatment of pAVMs has traditionally been performed as an adjunctive procedure prior to resective surgery and has been shown to reduce intraoperative blood loss and surgical complexity.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Embolisation with curative intent using Onyx® has recently been described in the treatment of superficial small- to medium-sized pAVMs (ruptured and unruptured), with a compact nidal architecture, one or two arterial feeding vessels and a well-demarcated venous drainage network. Most series have documented complete or near-complete occlusion rates with associated low morbidity and mortality rates.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9–12</span></a> The combination of embolisation agents such as Onyx® and more advanced endovascular methods such as the <span class="elsevierStyleItalic">pressure cooker</span> technique have helped to achieve higher occlusion rates.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> The use of Onyx® alone to treat an unruptured pAVM that has manifested as IH in the absence of previous hydrocephalus or haemorrhage has been documented in a single case, where they achieved complete exclusion of the lesion and resolution of the IH.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> After an exhaustive review of the literature, we can state with some confidence that ours is the first published case to document the combined use of this agent with the <span class="elsevierStyleItalic">pressure cooker</span> technique in this subgroup of patients (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). We achieved complete exclusion of the pAVM in a single session, resolution of IH and, above all, maintenance of clinical and radiological stability up to five years after the procedure.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Conclusion</span><p id="par0045" class="elsevierStylePara elsevierViewall">Embolisation with curative intent combining the <span class="elsevierStyleItalic">pressure cooker</span> technique and the embolising agent Onyx® is justified in cases such as that presented in this article, where a small, superficial, unruptured pAVM with a compact nidus and well-defined vascular afferents and efferents manifested clinically as IH in the absence of hydrocephalus or previous haemorrhage. Arteriographically, pial arterial contributions dependent on the right middle cerebral artery and the right posterior cerebral artery were identified. Venous drainage was located at the level of the superior sagittal sinus. An associated right transverse sinus stenosis was also identified. The existence of secondary intracranial hypertension was corroborated by monitoring with an intracranial pressure sensor. An interventional procedure was carried out consisting of embolization of the arterial supplies of the lesion using Onyx®. The clinical-radiological findings after the procedure were favorable: the papilledema disappeared and complete exclusion of the malformation was achieved. A new intracranial pressure measurement showed resolution of intracranial hypertension.
