Las malformaciones arteriovenosas (MAVs) durales representan del 10% al 15% del total de las MAVs intracraneales y entre ellas las localizadas en el tentorio constituyen un subgrupo minoritario. Sin embargo, en los últimos años se ha comprobado que estas malformaciones tienen una fuerte tendencia al sangrado, habiéndose establecido una viva controversia sobre cuál debe ser el método de tratamiento de estas lesiones.

Los autores presentan tres casos más de MAVs tentoriales. En dos pacientes la presentación clínica fue con sangrado intracraneal y en otro se produjo un déficit neurológico progresivo. Dos pacientes fueron intervenidos quirúrgicamente tras haber sido sometidos a embolización transarterial y el otro falleció por resangrado antes de que pudiera ser intervenido.

Se comentan el perfil clínico y el pronóstico de la MAVs tentoriales y las alternativas terapéuticas en los pacientes portadores de estas lesiones.

Dural arteriovenous malformations (DAVMs) represent 10% to 15% of all intracranial AVMs and those located at tentorial level are a minor subgroup. However, these dural AVMs carry a high risk for intracranial bleeding. During the last few years different therapy approaches to these vascular malformations have been proposed and there is a controversy regarding the best method of treatment.

The authors present three more cases of tentorial DAVMs. Two patients presented with intracranial bleeding and the other developed progressive neurological deficits. Two patients underwent surgical treatment and the other died as a result of rebleeding before he could be operated.

The clinical presentation, the final prognosis and therapy options in patients with tentorial DAVMs are commented.
