id="sect0025">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">The spinal segment more frequently affected by high-energy trauma is the cervical spine, about 6–15% of cervical spine injuries are cervical facet dislocations and bilateral dislocations are frequently associated with neurologic injury.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">1</span></a> According to Allen classification, cervical bilateral facet dislocations (CBFD) result of a mechanism of cervical spine hyperflexion and posterior distraction and they generally demonstrate 50% or more anterior displacement of the upper vertebral body over the lower. There is almost universal agreement regarding the need for surgical stabilization in cases of CBFD.</p><p id="par0010" class="elsevierStylePara elsevierViewall">The reported rate of failure to diagnosis cervical spine injuries ranges from 5% to 20%.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">2</span></a> Late diagnosis of CBFD (more than 3–8 weeks) is rarely described in the literature, which raises concerns in the management of these patients. The main issues are the risk of neurologic deterioration and treatment options, including reduction technique and different approaches combinations. In order to explore these questions, we present a literature review and the illustrative case of an old CBFD that was successfully treated 8 months after trauma. In our search only two authors<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">3,4</span></a> reported similar cases with the same or longer intervals between the injury and its treatment (8 and 14 months).</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Case report</span><p id="par0015" class="elsevierStylePara elsevierViewall">A 44-year-old healthy woman was involved in a road traffic accident. She was admitted at emergency department in another institution with neck pain and without neurological changes in examination. The cervical spine radiograph showed no fractures or dislocations. She was discharged with pain medication, and later oriented to physical therapy. After 8 months, she presented to our outpatient clinic with persistent and disabling neck pain with mechanical characteristics. The neurological examination was unremarkable. The cervical spine radiography (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) demonstrated an approximately 40% anterior subluxation of C5 vertebral body on C6 with bilateral C5-C6 facet dislocation (DFS3 – Allen classification), without signs of instability in flexion/extension views, and no significant in situ ossification in the computed tomography (CT) scan (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Magnetic resonance imaging (MRI) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>) showed obliteration of the anterior subarachnoid space due to prolapsed C5-C6 intervertebral disc, the spinal cord did not show signal change in any weighting. Following the clinical and imaging findings, this case scored the value of 6 in Subaxial Cervical Spine Injury Classification system (SLIC), so a strong recommendation for operative treatment was proposed.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">We opted for a combined “back-front-back” approach, beginning with partial bilateral resection of C5 and C6 facet joints by posterior approach, this allowed facet release for further reduction. After, we carried out an anterior C5-C6 discectomy and fusion with autograft (iliac crest) and plate. Successful reduction was achieved after disc space distraction. Lastly, the patient was repositioned prone and bilateral fixation with lateral mass screws and titanium bars was undertaken between C5 and C6. Surgery was performed under intraoperative neurophysiological monitoring (IONM) with somatosensory and motor evoked potentials (MEPs and SSEPs), which remained unchanged.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The postoperative period was uneventful. At 1 year follow-up, the patient maintains significant improvement in neck pain and no restrains in daily activities. Lateral radiographs of the cervical spine in flexion and extension (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>) showed no signs of instability and interbody fusion.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Discussion</span><p id="par0030" class="elsevierStylePara elsevierViewall">Bilateral facet dislocations at cervical subaxial levels are unstable injuries and commonly associated with neurological deficits. Early management of these cases is required to prevent the risk of neurological deterioration. However, some patients present minor clinical picture, which can justify the delay in diagnosis. The initial misdiagnosis of cervical spine injuries is not uncommon, even with the use of CT and MRI.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">5</span></a> In one study of 367 patients with cervical spine injuries, Platzer et al.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">2</span></a> found a diagnostic failure rate of 4.9%.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Concerning the reported cases of old CBFD<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">3,4,6–13</span></a> a literature search was made and significant findings are summarized in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. There is a clear male predominance and the age of most patients stands between thirty and fifty years. The most common levels of CBFD are C4-C5 and C5-C6 (about 80%). We report a case of a 44-year-old woman with C5-C6 CBFD treated 32 weeks after the traumatic episode. The majority (80%) of cases in the literature were managed before 12 weeks. We could only find one case treated after a similar interval since the injury<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> and a recent case report by Srivastava et al.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">4</span></a> treated after 98 weeks. As stated by in both papers, not always we are faced with cases of misdiagnosis. Additional reasons for treatment delays are conservative treatment attempt, loss to follow-up and demand for second opinion by the patient.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">Depending on the authors, the time to consider an injury as old or neglected ranges among 3,<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">7,9–11</span></a> 4<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">6,8</span></a> and 8<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> weeks. Regardless different criteria, in all cases reported in the literature the injuries were not treated at the right time and management options were restricted at the time of diagnosis. Delayed presentation of CBFD is a great challenge since the optimal treatment of these cases has not been established in the literature.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> The surgical dilemma comes from progressive deformity, bone fusion, fibrous tissue around facet and uncovertebral joints, soft tissues and ligaments disruption, articular surface remodelling and potential neurological deterioration. Non-operative treatment of these cases is associated with a high incidence of displacement.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">Several surgical methods have been reported to treat patients with old CBFD using a combination of anterior and posterior approaches, with instrumentation. The optimal treatment intends to obtain good sagittal alignment, decompression of the neural structures, solid fusion and pain improvement. To achieve anatomical reduction, a combined procedure is usually required. Closed reduction by traction is successful only in 20% of patients presenting more than 72<span class="elsevierStyleHsp" style=""></span>h after injury.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">14</span></a> Unlocking the facet joints from fibrous tissue around them to allow further reduction is reasonable as a first step, although reduction can generally be achieved only after distracting over the disc space, because of the resistance from the uncovertebral joints.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> From previous articles, only two<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">3,4</span></a> described the same surgical option for posterior-anterior-posterior. Bartels and Donk<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> report the first 2 cases with an initial anterior approach. In both, the attempts to reduce the dislocation after discectomy failed despite various manipulations. However, after posterior facet release, the reduction was not possible and satisfactory reduction was achieved only after disc space distraction. Based on this, they changed their strategy to posterior-anterior-posterior procedures, in which the facets are released and the dislocation is reduced after discectomy, after, posterior spine fixation is performed. This sequence reduces the changes in positioning (two instead of three). Srivastava et al. also described this option based in their literature review: old CBFD require at least two stages of surgery. Following the experience of Bartels and Donk<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a> and Payer and Tessitore<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">8</span></a> they decided to go posterior-anterior-posterior with a good result. Gonschorek,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">15</span></a> on his comment about this article also advise to follow this sequence, though the presence of significant disc herniation can be an indication for anterior approach first. Payer and Tessitore<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">8</span></a> treated one case with anterior-posterior-anterior approach, but the procedure presented difficulties for initial reduction and cage placement as noted before.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Hassan et al.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">7</span></a> reported a series of 10 patients with old CBFD treated initially with 1 week of skull traction. The reduction was achieved only in 2 patients, followed by anterior discectomy and fusion. One patient needed a posterior release with reduction followed by posterior fixation. Seven patients did not achieve reduction after facetectomy, another week of skull traction was applied followed by anterior discectomy and fusion. This larger group of patients ended up being submitted to a lengthy four-stage procedure. Madhavan et al.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">6</span></a> included 3 cases of unrecognized CBFD in a series of cervical spine injuries, they were identified after the treatment of cervical lesions in other levels, two cases were managed by revision of previous posterior approaches, one case was managed conservatively. The cases reported by Barrenechea et al.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">13</span></a> and Sanaullah et al.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">12</span></a> are both relatively recent, since they were treated 8 weeks after the injury. In the first, a single posterior approach under IONM was possible using a technique with reduction screws resembling the method used in high-grade lumbar spondylolisthesis. In the second, a single anterior approach was possible via anterior corpectomy. Liu et al.,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">9</span></a> Jain et al.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> and Goni et al.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">11</span></a> described posterior–anterior procedures in small series of CBFD. They reported successful reduction after posterior release and interspinous wiring for posterior fixation. They advocated a two-stage procedure as faster and with lower risk than a three-stage procedure. As described, satisfactory reduction is not assured by initial posterior approach, particularly in cases with longer delays and without uncovertebral joints release; moreover, interspinous wiring is not as solid as a lateral mass screws construction.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Overall, either our patient or other cases achieved a good neurological and biomechanical condition. These good results highlight the importance and safety of treating an old CBFD. The use of IONM has been used more often, as illustrated by recent articles.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">4,13</span></a> In multiple stage procedures, including reduction of a significant dislocation, changes in patient positioning and decompression of neural structures, IONM (particularly MEPs) improves the safety of procedure, and likely contributes to better results.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Conclusion</span><p id="par0060" class="elsevierStylePara elsevierViewall">Late surgical treatment of CBFD is safe and essential for the correction of deformity and decompression of the neurological structures, regardless the delay time. In these cases, multiple stages procedures must be considered for successful reduction and stabilization of the spine. To minimize the length of surgery and optimizing the procedure the option for posterior-anterior-posterior seems adequate. Surgery was performed under intraoperative neurophysiological monitoring.
