Spontaneously draining chronic subdural and subgaleal empyema. Case report

Summary

Spontaneously draining of chronic subdural and subgaleal empyema is an exceptional condition. We report a case of a 32-year-old Angolan man who was referred to us for diagnosis and treatment. Five years before admission he had suffered head injury. After this event, he progressively developed ambliopia, fluctualit bulkiness in the frontal region which sometimes spontaneously drained purulent secretion, and finally right hemiparesis. Computerized tomography (CT) showed a left subgaleal and subdural collection in the middle frontal region. A chronic calcified subdural empyema and the overlying bone flap were excised.

We emphasize the importance of an early diagnosis in order to reduce the high morbi-mortality associated with this condition.

El drenaje espontáneo de un empiema subdural y subgaleal crónico es un fenómeno excepcional. Los autores comunican el caso de un paciente angoleño de 32 años de edad, que les fue enviado para diagnóstico y tratamiento. Cinco años antes del ingreso, había sufrido un traumatismo craneoencefálico, tras el cual, desarrolló ambliopía progresiva, abultamiento fluctuante en región frontal, que en ocasiones drenó espontáneamente una secreción purulenta, y por último, hemiparesia derecha. La tomografía computerizada (TC) demostró la existencia de una colección subdural y subgaleal en región frontal izquierda. El paciente fue intervenido quirúrgicamente, evacuándose un empiema subdural crónico calcificado, y retirándose el colgajo óseo suprayacente.

Los autores desean enfatizar la importancia del diagnóstico precoz, con el fin de reducir la elevada morbi-mortalidad asociada a este proceso.
