elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Traumatic brain injury (TBI) has become an important matter in public health on a global level. It primarily affects young, economically active people of reproductive age, and is therefore one of the leading causes of loss of human potential.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">7</span></a> The incidence of TBI is around 200–600 cases for every 100,000 inhabitants.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">8–10</span></a> However, in middle- and low income- countries these figures are underestimated due to the paucity of epidemiological records.</p><p id="par0010" class="elsevierStylePara elsevierViewall">In developed countries, road traffic accidents are one of the most common causes of TBI. In contrast, in developing countries interpersonal violence accounts for a greater proportion (especially in Central America, South America, Central Africa and the Middle East), with gunshot wounds (GSWs) standing out in particular.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">5–7</span></a> This suggests that the causes and characteristics of TBI are related to the social context. Colombia, for example, is a country where GSWs stand out as a prevalent cause,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">11</span></a> especially in areas with a high rate of interpersonal violence, like for example the city of Cali.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">1–4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In the USA, in a series of patient at emergency departments in Indianapolis, a survival rate of 7–15%<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">12</span></a> was reported in patients with TBI due to a GSW. The majority of victims (approximately 90%) die before reaching the emergency department, while only 50% of those who make it to hospital are known to survive. It is estimated that in the USA, 70,000 people/year are victims of GSWs, resulting in 30,000 deaths.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">13–16</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Cali is one of the most violent cities in Latin America, with a murder rate of 53.7 for every 100,000 inhabitants in 2016.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">17</span></a> For 1996, a rate of 105 murders per 100,000 inhabitants was recorded, falling to 81 per 100,000 by 2010.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">17</span></a> In the majority of cases, the cause of death was GSW, resulting in a serious public health problem.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">18</span></a> However, there is a lack of analysis of the clinical characteristics and outcomes of patients with TBI due to a GSW.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The purpose of this study is to present statistics on patients with GSWs to the head admitted to the Fundación Valle del Lili in Cali, Colombia, with the aim of characterising the affected population in order to identify actions that might contribute to reducing morbidity and mortality and contribute to improving post-trauma functioning.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">A descriptive, observational, retrospective study was conducted by reviewing the digital and physical medical records of patients over 18 years of age with TBI secondary to a GSW in a civil setting who were admitted to the emergency department of the Fundación Valle del Lili in Cali, Colombia, between January 2010 and February 2016. The data were entered in the Fundación Valle del Lili's neurotrauma database in the general register of TBI in adults and children, which includes 364 patients admitted within the specified period. This study was approved by the institution's Ethics Committee. Due to the retrospective nature of the data collection, it was not necessary to obtain informed consent from the patients.</p><p id="par0035" class="elsevierStylePara elsevierViewall">For the statistical analysis, the numerical variables were presented using measures of central tendency and dispersion, depending on the distribution of the data, while the categorical variables were presented as proportions. The sample of patients was then compared based on clinical outcomes at discharge (death and disability). An adverse clinical outcome was defined as an Glasgow Outcome Scale - Extended (GOSE) of 3–6 at discharge. The Chi<span class="elsevierStyleSup">2</span> test was used to evaluate differences in categorical variables. For comparisons of continuous numerical variables, the two-sample <span class="elsevierStyleItalic">t</span>-test or the Mann–Whitney <span class="elsevierStyleItalic">U</span> test were used, depending on the distribution of the data. The Kaplan–Meier analysis was also used to estimate survival in the population studied. Results with a <span class="elsevierStyleItalic">p</span>-value <0.05 were defined as statistically significant. Data analysis was performed using the STATA 12.0 programme.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Demographic and clinical data</span><p id="par0040" class="elsevierStylePara elsevierViewall">A total of 95 patients were included in the analysis. Their demographic and clinical characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The average age of the patients was 28 (21.9–37.1); 91.5% were male; the majority of the injuries were due to assault (interpersonal violence) at 54.7%, followed by 26.3% of cases where the context was unknown and finally 13.6% were accidental injuries (“stray bullet/weapons handling”). Sixty-five patients were admitted to the emergency department by ambulance within the first hour following the trauma (arrival times range of 1–2<span class="elsevierStyleHsp" style=""></span>h). The Injury Severity Score (ISS) of the patients at the emergency department had a median of 48 (IQR 22–75) and the GCS on admission was between 3 and 8 in 64.2% of cases, 9–12 in 6.3% and 13–15 in 28.4%. In 86.3% of cases a simple head CT scan was performed and assessed based on the Marshall score, finding that 36.7% had a score of I–II, 24.2% III and 17.8% IV, while only 4.2% had scores of V–VI.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Mortality</span><p id="par0045" class="elsevierStylePara elsevierViewall">Mortality and the GOSE are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The variables associated with greater mortality are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. In particular, 97.7% of deceased patients had a GCS between 3 and 8, compared to 37.3% of survivors (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001); in contrast, 0% of deceased patients scored 13–15 on the GCS versus 52.9% of patients who survived (<span class="elsevierStyleItalic">p</span><<span class="elsevierStyleHsp" style=""></span>0.001). Likewise, the ISS and APACHE II values were found to be determiners of patient prognosis. The former had a median of 75 (IQR 75–75) in the group of deceased patients compared to a median of 26.5 (IQR 11–43) for survivors (<span class="elsevierStyleItalic">p</span><<span class="elsevierStyleHsp" style=""></span>0.001), while the latter had a median of 20 (IQR 17–23) in the deceased patients versus a median of 10 (IQR 8–15) in survivors (<span class="elsevierStyleItalic">p</span><<span class="elsevierStyleHsp" style=""></span>0.001). Blood pressure on admission showed no differences between patient groups.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">Another relevant finding was that the incidence of bilateral mydriasis was higher in the group of patients who died (45.2%), while patients who survived only presented this finding in 4% of cases (<span class="elsevierStyleItalic">p</span><<span class="elsevierStyleHsp" style=""></span>0.001). In contrast, patients who presented no response to light survived in 74.5% of cases, with only 16.7% of those with this finding dying. Of those patients who died, 71.4% had a unilateral pupillary reflex (<span class="elsevierStyleItalic">p</span><<span class="elsevierStyleHsp" style=""></span>0.001).</p><p id="par0055" class="elsevierStylePara elsevierViewall">Patients who did not require intubation in the emergency department, but who needed to be re-intubated had a mortality of 46.5% versus 78.8% in those who did not require intubation and were not re-intubated (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001); in contrast, patients who were intubated for the first time in the emergency department had a mortality of 53.5% with a survival rate of just 21.2% after hospital discharge (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001).</p><p id="par0060" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the findings for mortality versus the Marshall score in the CT. Some 17.3% of patients who survived required a craniectomy and 21.6% required surgery to remove fragments, while only 4.7% of those who died underwent surgical treatment. However, this was not statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.055). In total, 18 patients were operated on, 16 of whom survived, with only 2 dying. Of the 16 patients who had surgery and survived, only one patient developed a surgical site infection (SSI) associated with a craniectomy for the removal of fragments and drainage of a haematoma. One other patient developed an SSI associated with another type of surgery.</p><p id="par0065" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the data on length of stay and complications. Length of hospital stay was longer in patients who survived, with an average stay of 7 days (IQR 5–12.5), while patients who died had an average stay of one day (IQR 0–1). Intensive care unit (ICU) requirements were also greater in patients who survived (88.5%) versus (60.5%) in those who died (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002). As with time in hospital, time in the ICU was greater in those who survived, with an average of 5 days (IQR 2–9) versus one day (IQR 1–1) in those who died (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.0002). Mechanical ventilation requirements were higher in the group of deceased patients, at 96.2% of the total number of patients, compared to 63% of patients who survived (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002). Among survivors, the following healthcare-associated infections were presented: 3.8% SSI (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.502), 11.5% ventilator-associated pneumonia (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.033), 3.8% catheter-associated urinary tract infection (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.502), 1.9% bacteraemia (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.99) and 5.8% other nosocomial infections (osteomyelitis/tracheitis) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.249). Some 86.5% of surviving patients (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>45) received prophylactic antibiotic treatment due to their injury, regardless of whether they underwent surgery, versus 17.1% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>7) of deceased patients.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Adverse clinical outcome</span><p id="par0070" class="elsevierStylePara elsevierViewall">Lastly, only the group of survivors was analysed at discharge using the GOSE (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>). They were divided into two groups: those with moderate to severe disability (3–6), which were considered adverse clinical outcomes, and those with good clinical outcomes (7–8). From the variables compared, it was found that 52.4% of patients with a GOSE between 3 and 6 presented partially closed opening of the basal cisterns, while 84% of patients with a GOSE between 7 and 8 presented fully open basal cisterns. The APACHE II score was lower in patients with a GOSE of 7–8, with an average of 8 (IQR 6–13), compared to patients with a GOSE of 3–6 whose average was 11 (IQR 9–13). For the remaining variables evaluated, no statistically significant differences were found (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the Kaplan–Meier survival curve.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">In contrast to the epidemiology observed in other studies, where half of deaths due to TBI from a GSW were suicides, in our study the highest proportion of patients suffered this injury in the context of interpersonal violence. Moreover, we found accidental injury to be the third most common cause, in keeping with the literature reviewed.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">19–21</span></a> It can be seen that in both our study and the literature, the highest incidence of these lesions is presented in men and in the young, with an average age of between 28 and 30.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">20–28</span></a> Injuries in this working-age population can lead to disabilities in looking after oneself, performing activities of daily living and working, thereby generating sizeable losses for the country on a socioeconomic level. This series does not differ from the literature with regard to time of arrival (1<span class="elsevierStyleHsp" style=""></span>h) or mode of transport, medicalised ambulance being the most common.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">20</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">Many factors have been evaluated to define the greatest risks of adverse outcomes at discharge and death in patients with TBI due to GSW. There are reports that systolic blood pressure below 90<span class="elsevierStyleHsp" style=""></span>mmHg on admission is a factor in poor prognosis.<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">21,25</span></a> However, no statistically significant association between blood pressure on admission and mortality was seen in our study. Mortality in relation to GCS is similar to that reported.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">20,24,26,29</span></a> Because the majority of the patients who died did so in the first 24<span class="elsevierStyleHsp" style=""></span>h, it is assumed that this period is critical for defining patient prognosis and requirements for aggressive medical and surgical interventions.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">20–22,30</span></a> It can be observed that in developing countries such as India, mortality is much higher, reaching 87.69%, with greater mortality during transfer to medical centres, which could indicate that prognosis is influenced by the economic resources of the country where the incidents occur.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">31</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">A study of 107 cases by Ozkan et al. reveals a mortality rate of just 21%. On analysing the distribution of GCS scores on admission, it can be seen that 35 patients presented a GCS between 5 and 7, 37% of whom died, and 3 presented a GCS of 3, of whom 100% died. This supports our study's finding that a lower GCS score is linked to higher mortality.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">20</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Another variable for poor prognosis is an altered pupillary light reflex, which is often present in 50–60% of patients with TBI due to GSW. Our findings were in keeping with prior reports on this aspect.<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">21,26,30,32</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">GSWs to the head are considered contaminated wounds, since they involve necrotic brain tissue and retained bone fragments that act as infectious foci. These patients therefore require antibiotic prophylaxis.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">16,21</span></a> The primary infectious complication following a surgical procedure is surgical site infection. In the twentieth century, incidences between 5% and 23% were reported. This study found a low incidence of just 3.8% compared to that reported in the literature, taking into account that the vast majority of the patients did not undergo surgery. Another type of documented infections are those of the central nervous system, which have a high mortality rate<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">20,21</span></a>; none were presented in the 95 patients in this study. The patients who died had a mean length of hospital stay of one day, and, for this reason, they did not have the chance to develop an infection.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Various studies have found injury to the ventricular system and injury to both hemispheres to be poor prognostic factors. These two factors were not directly compared between deceased and surviving cases in our study. However, these findings could be correlated with Marshall scores<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">19,20,22,26,27</span></a> of III to IV, which indicate a diffuse lesion with uni- or bilateral cerebral oedema and were present to a greater extent in patients who died. Marshall scores of VI are associated with high mortality, which can be explained by the presence of undrained haematomas which are a poor prognostic factor.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">20,26</span></a> It has been seen that patients who survive often go on to have moderate or good functioning when follow-up is conducted with the GOSE.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">20,22,24</span></a> This was demonstrated in our study, with 54.1% of all patients obtaining a score of 7 or 8 (complete recovery without deficits).</p><p id="par0110" class="elsevierStylePara elsevierViewall">On cross-referencing mortality and ISS data, we can observe that those patients who presented a higher ISS had a higher rate of mortality. In other words, those patients with more than one compromised system had a higher risk of death.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">32</span></a> Regarding prognosis, two factors were found to be statistically significant in this series after discharge: occlusion of the basal cisterns and a higher APACHE II score.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conclusions</span><p id="par0115" class="elsevierStylePara elsevierViewall">Mortality secondary to penetrating TBI due to GSW is high, especially in developing countries such as Colombia. There is a need to identify early which patients may have a favourable neurological prognosis, with the aim of intensifying therapeutic efforts in these patients, thereby improving the neurological prognosis and reducing morbidity and mortality. A univariate analysis was performed to determine factors associated with death and adverse clinical outcomes.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">A total of 95 patients older than 18 years, with traumatic brain injury by gunshot were included in the civil context. The 91.6% were male. The main context was interpersonal violence with 54.7%. The most common method of transportation was by ambulance (79%). The Glasgow score at admission was 3–8 in 64.2% of cases; 9–12 in 6.32% and 13–15 in 28.4%. On admission, head CT scan was performed in 82 (86.3%) patients within the first hour, finding a Marshall-Score between I–III in 60.9%, of IV in 17.8% of cases and a score between V–VI and in 4.1%. The trajectory was non-transfixing penetrating in 43.2%, transfixing in 27.3% and tangential in 9.5%. La trayectoria era penetrante no transfixiante en el 43,2%, transfixiante en el 27,4% y tangencial en el 9,5%.
