Symptomatic thrombosed venous aneurysm after stereotactic radiosurgery for brain arteriovenous malformation: Case report of a late complication mimicking radiation necrosis

Abstract

Late complications of radiosurgery for brain arteriovenous malformation include cystic formation, chronic encapsulated hematoma and radiation-induced tumors. Other complications are rarely reported.

We present a case of an adult patient who received radiosurgery for treatment of an unruptured parietal arteriovenous malformation. He was followed-up for 10 years and angiographic cure was documented. Fifteen-years after radiosurgery, he complained of new-onset progressive focal seizures associated with a thrombosed venous aneurysm with persistent arteriovenous shunt angiographically occult but evidenced intraoperatively. After resection of the lesion, symptoms disappeared.

This case depicts a symptomatic thrombosed venous aneurysm presenting as a rare delayed complication of radiosurgery for the treatment of brain arteriovenous malformation, which can be associated with angiographically-occult persistent arteriovenous shunt.

Resumen

Las complicaciones de la radiocirugía como tratamiento de la malformación arteriovenosa cerebral incluyen las formaciones quísticas, el hematoma crónico encapsulado y los tumores secundarios a radiación. Otras complicaciones son raramente reportadas.

En el presente artículo presentamos el caso de un paciente adulto que recibió radiocirugía para el tratamiento de una malformación arteriovenosa parietal no rota. Tras un seguimiento de 10 años, se documentó la cura angiográfica. Tras 15 años post-radiocirugía, el paciente presentó convulsiones focales progresivas de nuevo inicio, asociadas a un aneurisma venoso trombosado con shunt arteriovenoso persistente angiográficamente invisible, pero evidenciado en el intraoperatorio. Tras la resección de la lesión, los síntomas desaparecieron.

Este reporte ilustra un aneurisma venoso trombosado sintomático que se presenta como una muy rara complicación tardía de la radiocirugía en el tratamiento de la malformación arteriovenosa cerebral, que puede estar asociado con persistencia del shunt arteriovenoso no visible angiográficamente.
