(C) Determination of the transverse diameter, defined as the distance between the medial borders of the pedicles at the mid-pedicle level.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Corpectomy consists of resecting the vertebral body in order to achieve anterior decompression of the spinal canal and dural sac, as well as stabilisation and adequate alignment of the vertebral column. Spinal cord compression can be attributed to multiple causes. Therefore, corpectomy may be useful in the treatment of various diseases, among which the following should be noted: primary or secondary tumours; infections; traumatic injuries; and deformities.</p><p id="par0010" class="elsevierStylePara elsevierViewall">The first case of hemivertebra resection was published by Royle<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">1</span></a> in 1928, for the treatment of congenital scoliosis, although the procedure was later abandoned due to the associated complications. In 1956, it was Hodgson and Stock who brought back the anterior approach to the column for the decompression of paraplegic patients with Pott's disease.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">2</span></a> In the 1960s, the anterior approach to the thoracolumbar spine became more common in degenerative diseases due to Sacks,<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">3</span></a> and in the series published by Leatherman and Dickson in the treatment of congenital deformities of the thoracic and lumbar spine with resection of the vertebral body.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">4</span></a> At the end of the 1980s, Harrington, for his part, published his series using an anterior approach on vertebral metastases with secondary fractures.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">5</span></a> Finally, the work by Tomita et al.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">6</span></a> is worth highlighting, who in 1997 described en bloc spondylectomy for the first time following the treatment of seven patients with aggressive tumours, and who in later years showed the positive outcomes of said approach for the treatment of such lesions in a large case series.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">7,8</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Meanwhile, in recent years, progress in fixation systems, improvements in titanium implant systems, the introduction of posterior techniques and the incorporation of spinal neuronavigation have enabled greater surgical development of the technique, with improved patient outcomes and reduced morbidity and mortality<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">9–11</span></a> (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">The objective of our study focuses on three elements. Firstly, to conduct a retrospective descriptive study on patients who underwent dorsal corpectomy at our site in the last 10 years; to determine how radiological parameters improve among said patients after surgery; and, finally, to determine what effects the above-mentioned improvements have had on operative outcomes, comparing the first five years of the study to the last five.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">We present a retrospective study of patients who underwent either dorsal or dorsolumbar junction corpectomy via the anterior or posterior approach from 2005 to 2015 at the Hospital Universitario de La Princesa. To that effect, all cases where said technique was applied were reviewed, noting the patients’ demographic characteristics, the type of lesion and implant, as well as any complications, peri-operative mortality and pre- and post-operative radiological characteristics. The patients’ general status was assessed according to the Karnofsky scale and the neurological assessment was performed in accordance with the American Spinal Injury Association (ASIA) classification.</p><p id="par0030" class="elsevierStylePara elsevierViewall">In all cases, the patients presented with radiological and/or clinical spinal cord compression seen in pre-operative magnetic resonance imaging (MRI), and after surgery, the decompression obtained was viewed with another MRI scan or computed tomography (CT). The patients underwent surgery in order to resolve epidural compression and/or maintain dorsal column stability. The patients with metastatic compression were assessed and treated according to the Tokuhashi Score recommendations, with surgery being indicated in those who scored above 9 in cases of single lesions and in the absence of other metastases.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Following the recommendations of the Spine Trauma Study Group, the radiological parameters studied were the Cobb angle to determine sagittal alignment and spinal canal area, by studying the dimensions thereof.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">12</span></a> The Cobb angle is defined as the angle formed between a line parallel to the superior endplate of one vertebra above the fracture and a line parallel to the inferior endplate of the fracture. The spinal canal area, on the other hand, was determined on the axial slice where there was the greatest degree of compression. Once the slice had been chosen, the product of the anteroposterior diameter—defined as the distance between the edge of the posterior wall of the vertebral body and the anterior border of the lamina of the vertebral arch—was calculated in millimetres, as well as the transverse diameter, which is the distance between the medial edges of the pedicles at the mid-pedicle level (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">The approaches used were anterolateral techniques, via the extrapleural transthoracic approach or a thoraco-phreno-laparotomy for lesions at the thoracodorsal junction, or posterior via the transpedicular approach. Among the patients who were operated on from 2008 onwards, surgery was guided with spinal neuronavigation (Brainlab) on a Jackson radiolucent table. In order to stabilise the vertebral bodies, autografts or titanium implants (VBR<span class="elsevierStyleSup">®</span> model by Ulrich medical, Gencel) were used at the level of the vertebral body (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>), alongside transpedicular or vertebral screws (Zodiac<span class="elsevierStyleSup">®</span> model by Prim).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">The statistical analysis was performed using the SPSS 21.0 statistics software. To that effect, non-parametric tests were used, applying the Wilcoxon signed-rank test for paired quantitative dichotomous variables in the analysis of the spinal canal area and angle measurements. In the case of qualitative variables, the Chi square test was used. On analysing clinical and radiological improvement, dichotomous variables were created for improvement in the ASIA grade, and area increase or angle reduction, with values of 120<span class="elsevierStyleHsp" style=""></span>mm<span class="elsevierStyleSup">2</span> and 4° being used. Finally, the patients were grouped together by differentiating those who were operated on in the first five years from those who were operated on in the later period, and they were compared using quantitative non-parametric tests with morphometric characteristics.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">Twenty-four patients were operated on, 14 of whom were male (58.3%), with a mean age of 52 (22–82). A neurological deficit was the predominant symptom, affecting 58.3% of cases, followed by pain in 37.5%. The mean pre-operative Karnofsky score was 68.3, ranging from 30 to 90 points, and the mean follow-up lasted 19.5 months (median: 8.5 months; range: 2–114 months).</p><p id="par0055" class="elsevierStylePara elsevierViewall">Regarding aetiology, 13 cases (54.2%) were attributed to a metastatic cause or a primary tumour, 6 to injury (25%) and 5 to infection (20.8%). Prior to surgery, the ASIA classification ranked three patients as grade A, two as grade B, five as grade <span class="elsevierStyleSmallCaps">C</span>, three as grade D and 11 as grade E.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Among the cancer-related causes, lung tumours were most common, affecting five patients (38.4%), followed by breast cancer in four cases (30.8%), and one of each of the following: kidney cancer; myeloma; melanoma; colorectal cancer; and primary leiomyosarcoma (7.7% in each case). With respect to the infectious aetiology, two cases were culture-negative, while the remaining three presented methicillin-resistant <span class="elsevierStyleItalic">Staphylococcus aureus</span> (MRSA), an echinococcal cyst and tuberculosis, respectively. The four injury cases were burst fractures (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">As regards approach, an anterior approach was used in 14 cases, eight of whom underwent thoraco-phreno-laparotomy for low thoracic lesions or lesions of the thoracolumbar junction. A transthoracic retropleural approach was performed on six occasions. The 10 patients who were operated on via the posterior approach all underwent the transpedicular approach and a posterior corpectomy (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>).</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Morbidity, understood to mean complications in the first 30 days, reached 25%, involving six cases. Two of these were pneumonia, another two were wound infection, one was symptomatic pleural effusion and one pulmonary embolism (PE), with all cases demonstrating a favourable evolution following medical treatment. There were no cases of death in the immediate post-operative period.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The radiological results studied were obtained using pre- and post-operative MRI or CT according to the indications of the Spine Trauma Study Group,<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">12</span></a> as described in the previous section. Following the statistical analysis, a significant improvement was seen both in the Cobb angle and spinal canal area compared to pre-surgery values. In the first case, a mean kyphosis correction of 4.18° was observed, measured by means of the patient's current Cobb angle versus the initial value (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). As regards spinal canal area, a mean increase of 130.8<span class="elsevierStyleHsp" style=""></span>mm<span class="elsevierStyleSup">2</span> (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) was observed (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">In relation to the clinical results, we observed that 11 patients (45.8%) improved by at least one point on the ASIA scale (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), taking into account that 12 patients (50%) were initially in grade E. One case (4.2%), initially classified as ASIA grade A, showed no improvement. Moreover, of the 11 patients who showed neurological improvement, 33.3% retained the ability to walk (<a class="elsevierStyleCrossRef" href="#fig0025">Fig. 5</a>). As regards clinical status, measured according to the Karnofsky scale, a significant mean improvement of 13 points was also observed, compared to the previous score (68.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>20.34 vs. 81.67<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>20.57, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="fig0025"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">However, when we tried to correlate these clinical data with those obtained on studying the above-mentioned radiological parameters, we found no relationship between them. In this case, the values 4° and 130<span class="elsevierStyleHsp" style=""></span>mm<span class="elsevierStyleSup">2</span> were used, since they were the mean values. On comparing said values together with the improvement seen in the ASIA classification, no significant difference was observed between the two groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.1 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.69, respectively).</p><p id="par0090" class="elsevierStylePara elsevierViewall">Finally, on studying the group of patients operated on between 2005–2010 and those who underwent surgery in the last five years of the study, no difference was observed in the results obtained, neither in the radiological nor clinical characteristics. It was observed, however, that there has been a shift in technique in the last five years in favour of the posterior approach, which has been more frequently used in place of anterior techniques. Between 2010 and 2015, thoraco-phreno-laparotomy was used for four lesions at the thoracolumbar junction, mainly at the L1–L2 level, while the posterior approach was more common at level D11–D12 (3 of the 4 cases [75%] at said level in this period). This shift is most significant when we analyse the remaining thoracic vertebrae, since between 2005 and 2010 eight patients underwent anterior approaches, while in the last five years of the study all cases except one (11 of 12 patients) were operated on via posterior approaches.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">Corpectomy is a standard technique in complex spinal surgery that enables decompression and stabilisation of the vertebral column in the anterior region. One of the objectives of this study was to determine the level of decompression obtained with the technique and to correlate it with the relevant clinical outcomes. However, one of the limitations we faced on comparing our data with those published previously was that, in the literature, cases are mostly grouped by aetiology, rather than location, thus hindering comparison with similar studies. In our series we can observe how, given the lack of polytrauma patients seen, corpectomies indicated for fractures were fewer in number than we expected, and that, compared to metastases, they were considerably less common. In our case, therefore, metastasis was the most frequent aetiology, followed by fractures and infection, which in terms of incidence is usually far less common. As regards the causes behind these processes, it is worth highlighting that, as we found in the literature, the most prevalent were lung metastases.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">13</span></a> However, in the infectious aetiology, it is again notable that, while in most of the series published on spondylodiscitis, the most common microorganism is MRSA, in ours the latter is observed with the same frequency as echinococcal cyst and <span class="elsevierStyleItalic">Mycobacterium tuberculosis</span>, which is again explained by the small sample size and the presence of two culture-negative cases.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">14</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">Focusing on the morbidity and mortality associated with the technique, again the series present aetiology-related differences; in our series we found a morbidity rate of 25% along with 0% mortality. In the treatment of metastases—the main cause in our case—in a meta-analysis by Kim et al.,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">15</span></a> who reviewed 2495 patients from 33 different studies, the authors found a mean complication rate of 29% (range from 5 to 65%) and 5% mortality (range from 0 to 22%), which are very similar to the figures found in our study. Similar results can be found for infections and injuries.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">16,17</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">There are very few series analysing radiological parameters following this type of surgery; the most frequently-analysed morphometric parameter in previous studies is the Cobb angle. In our case, the difference obtained between pre- and post-operative parameters was 4.18°, which is similar to the findings of Oskouian et al., but somewhat smaller than the differences observed in other articles, where mean corrections of up to 9° are described.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">18–20</span></a> Spinal canal area, on the other hand, has been studied to an even lesser extent. In our case, we obtained a canal size improvement of 40%, similar to the findings of Oskouian et al., but somewhat smaller than the improvement described by Lemons et al., of up to 80%.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">18,20</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">As regards the clinical improvements seen, in our case 45.8% of the patients showed an improvement in their ASIA grade and a 13-point improvement was seen in the Karnofsky scale. Similar results are also observed in other series, such as those described by Choi et al., in the treatment of fractures,<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">21</span></a> and Korovessis et al. and d’Aliberti et al. in spondylodiscitis.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">14,22</span></a> Meanwhile, in the series described by Oskouian et al., the rate of patients whose ASIA grade improved was around 25%,<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">18</span></a> although previous series had described improvements in up to 60%.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">23</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">Finally, as stated, no relationship was observed between improvements in radiological and clinical parameters. Lemons et al. and Sasso et al. had already described this phenomenon, basing it on the presence of two different processes. On the one hand, initial traumatic spinal cord injury leads to contusion of the glial and neural tissue, which continues with compression caused by the fracture or mass occupying the canal, facilitating the maintenance of inflammatory mechanisms, haemorrhagic necrosis, oedema and the onset of neurotransmitter toxicity, which activates a chain of inflammatory events and triggers necrotic cell death. As a result, the decompressive capacity of removing said mass may prevent the exacerbation of the neurological injury, even if no action is taken on the initial traumatic injury itself.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">20,24,25</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">This led us to analyse articles comparing the decompressive capacity of new techniques, such as the posterior approach, and minimally invasive techniques,<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">26</span></a> which seem to generate less blood loss and a shorter operative time, and which, despite having a smaller decompressive capacity, do not impede neurological improvement, as observed in the meta-analysis published by Xu et al.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">27</span></a> As we have seen in our study, a tendency towards a greater use of posterior techniques is observed, particularly since the introduction of neuronavigation systems. One example is the previous experience of our group, which published a series with low morbidity and mortality rates and excellent surgical outcomes (disappearance of pain when standing in 83% of cases and improved neurological deficits in 50%), all operated on via the anterior approach,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">11</span></a> as in other groups within our field.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">28</span></a> However, in the last five years, we have observed increased use of the posterior approach, not only in corpectomies, but also in other procedures,<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">29</span></a> with a similar or lower morbidity rate than that observed in previous series.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0125" class="elsevierStylePara elsevierViewall">Dorsal corpectomy enables significant spinal decompression, with improvements in neurological symptoms and patients’ ability to walk, as well as the stabilisation of the affected segment. However, there is a dissociation between clinical and radiological parameters, with no statistical relationship observed between them.
